Learning Disability Weight Management Service

Who to refer:

  • The person has a learning disability
  • Adults 16 +
  • BMI>30
  • Able to attend clinic independently/or with support
  • Able to give consent or/under guardianship

Referrals will not be accepted for patients that have full support packages and carer training will be available for identified providers.

It is important to explain clearly the issue that has led to the referral. This will enable us to assess the level of urgency of the referral and prioritise clinic

In order to help us prioritise referrals, please indicate in the referral any appropriate concerns:-

  • Is the person’s current day or residential placement impacting on their weight.
  • Is there a risk of self harm i.e. obesity/co morbid health needs
  • Is there a risk of neglect or abuse i.e. lack of understanding by those who support the person/ not following healthy lifestyle plans
  • Are there significant changes in the person’s health status
  • Are there significant changes in the person’s behaviour
  • Does the person have adequate support networks

What Happens:

Referrals will be screened by the Weight Management service and will be forwarded to the appropriate Community Learning Disability Nursing Team
