Midlothian HSCP

Health care professionals requiring community input for Midlothian patient should contact the Midlothian Single Point of Access (part of Midlothian Flow Hub).

The Single Point of Access is currently open from 9am to 5pm, Monday to Friday & 9am to 2pm Saturday and Sunday. This service will liaise with any of our Intermediate Care Teams to respond to your referral as appropriate. We can provide the following, some within 24hrs if required:

  • Physiotherapy – assessment, falls reviews, provide mobility aids, advice, exercises
  • Occupational therapy – assessment, falls reviews, provide equipment to aid independence, advice
  • Care at home – from 15 minute care visits to prompt client with medication to fully assessed large packages of care to keep clients safe in their own home
  • Social Work input – urgent assessments and reviews of clients in crisis at home, to prevent social hospital admissions
  • Liaising with medical/nursing teams to prevent hospital admission and deliver treatment at home
  • A place of safety for clients in crisis, when all options for community support have been exhausted

We will be communicating directly with the teams you may have had contact with before but aim to prevent professionals calling multiple teams to access the correct care for your patient.

We have links with all Midlothian Intermediate Care Teams: Rapid Response, Discharge to Assess, Community Respiratory Team, MSK Physiotherapy, Hospital at Home, Homecare, District Nurses, Duty Social Work, Dementia Team, etc.

​Who to refer:

Refer patients where rapid response is required to support patients who present with:

  • Inability to transfer/ mobilise to toilet or attend to personal care
  • Inability to access food and drink
  • Inability to self-manage medication
  • Rapid decline in cognition or social situation resulting in vulnerability in current home.
  • Resident in Midlothian

Who not to refer :

Please note –  We are not a medical service and while we can liaise with all the above teams to coordinate the correct care for Midlothian patients we do not take urgent referrals for the following services:

  • Mental Health (contact IHTT until 8am to midnight, and MHAS from midnight until 8am)
  • Hospital at Home (please use their current referral pathways)
  • Community Respiratory Team
