Weight Management Service Adult

Adult Weight Management and T2DM prevention

The Adult Weight Management and Type 2 Diabetes Prevention Team are available to support patients who would like to lose weight and make healthy lifestyle change.

The team can also support those at risk of developing T2DM or who have recently been diagnosed with T2DM and are keen to seek remission.

Weight Management

This service is available to patients who have a BMI >25kg/m2 irrespective of co-morbidities who want to lose weight and need support to make healthy changes.  After referral, patients will be triaged according to which programme they are likely to suit best.

Let’s Prevent

This is for patients who have pre-diabetes and are at risk of developing Type 2 Diabetes. Their HbA1c should be between 42 – 47mmol/mol. 

Remission

If diagnosed with T2DM in the last 6 years, patients may wish to explore aiming for remission (ie to get their HbA1c below 48). Further information about how to achieve this is available on our website.

DESMOND

This is an educational programme for those newly diagnosed with T2DM

Who to refer:

Adult Weight Management

  • Over 18 years old and BMI >25kg/m2

Let’s Prevent Diabetes

A diagnosis of pre-diabetes:

  • Hba1c 42-47 mmol/mol
  • Impaired FBG (5.5-6.9mmol/I)
  • Impaired GTT
  • At moderate or high risk of Type 2 Diabetes on completion of the Diabetes UK Know Your Risk Tool

Remission

  • Diagnosed with Type 2 Diabetes within the last 6 years 
  • BMI >27kg/m2 (no upper BMI limit) 
  • Aged 18-65 years 
  • Diet controlled with HbA1c >48mmol/mol, or on oral agents e.g. metformin with HbA1c >43mmol/mol 

DESMOND

  • A new diagnosis of Type 2 Diabetes within the last 12 months

Who not to refer:

  • Patients who:
    • Have a diagnosis of current eating disorder or potential binge eating disorder. Please see the Cullen Centre Ref Help page for further details.
    • Have uncontrolled hypothyroidism
    • Have unstable psychosis or severe and unstable personality disorder
    • Have current alcohol or drug dependency
    • Are pregnant (If BMI over 40kg/m2 can be referred by midwives to Metabolic Antenatal Clinic)
    • Have dementia
    • Have a moderate to severe learning disability (please refer to the Learning Disability team)


Bariatric Surgery

There is no direct referral into the bariatric surgical service. All patients who wish to be considered for bariatric surgery are required to successfully complete the Tier 3 weight management programme.

The weight management service will then refer patients to the surgical service as appropriate. Inclusion and exclusion criteria for the bariatric service can be found on the Bariatric Surgery Intranet page.

How to refer:

Referrals are accepted from all health care professionals via SCI-Gateway or patients/carers can refer using the self-referral forms below.

SCI Gateway:

For Adult Weight Management Service or Let’s Prevent for T2DM Prevention or T2DM Remission the referrals all come via the same SCI Gateway pathway:

AHP – Dietetics >> Weight Management >> LI Weight Mx – T2DM Remission

For DESMOND the referral pathway is:

SJH or WGH >> Diabetes Type 2 Education (Desmond) >> LI Type 2 Education (Desmond)

3.    Type 2 Diabetes Newly Diagnosed (DESMOND)
