East Lothian Prevention of Admission (POA)

The East Lothian Prevention of Admission pathway is hosted within the East Lothian Rehabilitation Service (ELRS) and is delivered by a team of Physiotherapy and Occupational Therapy practitioners based within the community.

The team’s role is to support patients from a therapy perspective who are at risk of admission within the next 24 hours and for whom the GP is happy to manage medical needs at home. A comprehensive clinical assessment will be completed, and any interventions provided, aiming to manage the patient safely at home.

The core hours of the service are Monday – Friday 8am – 4pm.

Currently there is no provision to accept referrals out of hours.

Interventions provided include:

  • Transfer assessment and provision of aids
  • Mobility assessment and provision of aids
  • Assessment of activities of daily living (either independently or with support of carers/family)
  • Graded practice of selected activities to improve function
  • Onward referral for package of care as indicated
  • Onward referral/link with community services as indicated

Who can refer:

GPs, DN’s, ANP’s

For same day assessment, referrals MUST be received before 2pm.

Who to refer:

Residents of East Lothian who are deemed to be at risk of hospital admission but can be medically managed at home.

Who not to refer:

Patients who are not at immediate risk of admission – please refer for urgent domiciliary physiotherapy via SCI Gateway for review within 5 days and/or Community OT for urgent input.

Patients with potential undiagnosed significant injury (e.g. acute onset significant back pain, at risk of fracture).
