West Lothian Community Physiotherapy (Domiciliary)

Information 

The West Lothian Community Physiotherapy team’s role is to support patients with physiotherapy input, in their own environment, when they are unable to attend an outpatient clinic.  

This is for patients that require slow stream rehabilitation for conditions such as:  

  • Falls (non–urgent) 
  • Neurological conditions 
  • Frail/elderly 
  • Orthopaedics 
  • Deconditioning/general reduced mobility 
  • Long term conditions 

This support may include assessment of joint movement, limb strength, mobility and transfers with subsequent provision of exercise programmes, advice and education, walking aids and other equipment to maintain the patient’s abilities to complete essential daily activities at home (either independently or with support of carers/family). Where possible as part of the patients’ physiotherapy intervention, self management programmes will be devised and encouraged. The community physiotherapy service works toward patient specific goals but would not provide long term, ongoing physiotherapy for maintenance. 

The core hours of the service are Monday – Friday 8.45 am – 4.45pm 

Who can refer: 

  • GP 
  • AHP’s 
  • DN’s 
  • Consultants 
  • Other support services eg. social worker, dementia link workers, nursing homes 

Who to refer:  

  • Housebound/unable to attend clinic by any method 
  • Requires an assessment within their own environment due to clinical need  
  • Only requires physiotherapy input  
  • Patient aware of referral and is able to participate in physiotherapy or has support to be able to participate 
  • Resident in West Lothian 
  • Over 16 years old 

Who not to refer: 

  • Patients who can attend an outpatient clinic for physiotherapy 
  • Patients who are acutely unwell and at immediate risk of admission* 
  • Requires urgent assessment* 
  • Requires intense rehab (> 2 visits per week)* 
  • Requires respiratory assessment*
