Edinburgh Community Diabetes Service

Information

The Community DSN service covers the City of Edinburgh only, acting as an interface between primary and secondary care diabetes services by visiting people at home to provide advice and guidance regarding their diabetes management. The service is aimed at people with poorly controlled diabetes and those at high risk of hospital admission.

The service is available Monday to Friday 8:00 AM to 6:00 PM (including most public holidays).

Please follow the Edinburgh Community DSN Referral Flow Chart below to ensure the Community Diabetes team is the correct service for your patient.

Who can refer:

Referrals are welcome from all Healthcare Professionals and are triaged and prioritised according to need and workload capacity.  

How to refer:

Referrals can be submitted via SCI-Gateway or by completing the DSN referral form which should be sent to us via email, along with a patient summary sheet to loth.communitydiabetesteam@nhs.scot

During acute illness, please contact the on-call diabetes registrar.
