The West Lothian Community Physiotherapy team’s role is to support patients with physiotherapy input, in their own environment, when they are unable to attend an outpatient clinic.
This is for patients that require slow stream rehabilitation for conditions such as:
- Falls (non–urgent)
- Neurological conditions
- Frail/elderly
- Orthopaedics
- Deconditioning/general reduced mobility
- Long term conditions
This support may include assessment of joint movement, limb strength, mobility and transfers with subsequent provision of exercise programmes, advice and education, walking aids and other equipment to maintain the patient’s abilities to complete essential daily activities at home (either independently or with support of carers/family). Where possible as part of the patients’ physiotherapy intervention, self management programmes will be devised and encouraged. The community physiotherapy service works toward patient specific goals but would not provide long term, ongoing physiotherapy for maintenance.
The core hours of the service are Monday – Friday 8.45 am – 4.45pm
