Hospice Care @ Home Daytime Service (Fast track)
Provides personal and emotional care to patients and their families in their own home for up to two visits during the day/evening.
- Patients can be referred from any health care professional but must be known to their District Nurse team.
- Patients can be referred to the service for end of life care, carer/family distress, or urgent palliative care need/crisis.
- Referrals are prioritised and care allocated by Registered Nurses as appropriate.
- This service is provided by Health Care Assistants.
- Shift times are from 8am – 9pm.
- Caseloads will be reviewed by a Registered Nurse and patients may be discharged.
- Rereferrals will be considered.
Hospice Care @ Home Overnight Service (Managed Care)
Provides nursing, personal and emotional care to patients and their families in their own home
- Patients can be referred to the service for end of life care, carer/family distress within the last six months of life.
- Referrals are prioritised for care using a prioritisation tool and care allocated by Registered Nurses as appropriate.
- This service is predominantly provided over night from 10pm until 7am by HCAs with some daytime Registered Nurse capacity as appropriate.
- Patients must be known to their District Nursing team
Urgent Hospice Care @ Home (Rapid Response)
Provides responsive person-centred care at home throughout the night 21:00 – 09:00
- Two teams (one Registered Nurse and One Health Care Assistants) work closely with LUCS to prevent crisis admissions, provide symptom management and support patients and their families.
- Urgent visit requests come via the LUCS team (DNs, GPs), Scottish Ambulance Paramedics, Care Home staff or directly from patients who have previously accessed the service or are known to one of the Lothian community specialist teams.
- Contact number has been provided to all care homes in Lothian offering support and visit overnight if appropriate.
Who can refer:
Referrals can be submitted via patient’s district nursing team but can also be done by another healthcare professional (ideally in discussion with the DN team)
Rapid Response urgent visits can be requested by LUCS team (DNs, GPs), Scottish Ambulance Paramedics, Care Home staff, or directly from patients who have previously accessed the service or are known to one of the Lothian community specialist teams.
Who to refer:
Daytime Service (Fast track): Patients can be referred to the service for end of life care, carer/family distress, or urgent palliative care need / crisis. This is a short term service up to 14days
Overnight Service (Managed Care) Patients can be referred to the service for end of life care, carer/family distress within the last six months of life.
Who not to refer:
For referrals to Overnight Service (Managed Care),patients must be known to their DN team
How to refer:
Referrals can be discussed with the Scotland co-ordination Hub by calling 0141 473 1785. (8am-10pm)
By completing the hospice referral form and returning via email to ci.mariecurie.sccreferrals@nhs.scot (Mailbox checked between 8am and 6pm Monday to Sunday)
