- This pathway is delivered from the 4 locality hubs within EdinburghHSCP (NE, NW, SE & SW)
- 2 referral slots a day per locality hub are available from Monday to Friday.
- Referral Times are 8.30am – 2pm.
Who to refer:
Rapid response required to support patients who present with:
- Inability to transfer/ mobilise to toilet or attend to personal care
- Inability to access food and drink
- Inability to self-manage medication
- Rapid decline in cognition or social situation resulting in vulnerability in current home.
- Resident in Edinburgh
The above criteria may also include a response to falls that present in a deterioration in function as described above
Who not to refer:
- Prevention of admissions that could be responded to the next day. For these referrals please use existing Social Care Direct referral route.
