Quads or Patella Tendon Rupture

History

  • History of fall with hyper-flexion of knee
  • Not necessarily painful after event
  • Swelling
  • Bruising
  • Abnormal patella position – high or low
  • Unable to straighten leg actively
  • Quads dysfunction
  • Difficulty with stairs

Signs

  • Limp
  • Swelling
  • Deformity – possible divot in distal quads/defect below patella
  • Bruising
  • Unable to straight leg raise

Secondary care referral

  • Urgent referral – may need urgent surgical repair

Primary care investigations

  • X-Ray – A-P and lateral of both knees for comparison of patella position
