The Older People’s Community Mental Health Team works with people who are over 65 years of age and may be experiencing, or are at risk of, or are recovering from an episode of mental illness.
The service provides various services including:
- Acute Out Reach/ Crisis Care
- Memory Assessments
- Memory Treatment Service
- Post Diagnostic Support Team

Single point of referral
All referrals should be made using the appropriate referral protocol on SCI Gateway.Referrers should note that patients will be triaged to a range of appropriate professionals depending on their needs / reason for referral. Patients should be made aware of this when being referred. If a patient has been discharged from the service they will need to be re referred using the SCI Gateway process.
Urgent Referrals
Where a referral is deemed urgent, this will be triaged by the team and a CPNe will be allocated to contact the patient and arrange visit within 3 working days.
Routine referrals
Routine referrals (including memory assessments) will usually be seen within 12 weeks of referral. The patient (or if appropriate, their relative) will be contacted to arrange a suitable appointment date.
Emergencies Referrals
Where the patient requires to be seen on the same day as an emergency between 08.30-16:30 please contact the Duty Nurse by telephone.
