Leith Community Treatment Centre – Older Peoples’ Rehabilitation and Assessment (OPRA)

Services

OPRA provides comprehensive assessment of frail older people. This is an outpatient day hospital service, which may involve one or several weekly attendances.  It is run between 9-5pm Monday to Friday.  Patients attend for a whole morning and return visits are scheduled as necessary.  Ambulance transport is available to and from appointments.

Who to refer :

  • Patients who are over 65 years of age with rehabilitation needs living in the North East of Edinburgh

Those for whom urgent Day Hospital attendance may avoid unscheduled admission

  • Patients who require ongoing care to facilitate early hospital discharge

Acute reduction in mobility or function without obvious injury

  • Frequent falls

Exacerbation of chronic medical problems which would benefit from specialist medical and multidisciplinary care

  • Those with Parkinson’s or other chronic health conditions with associated disability

Who not to refer:

  • Unstable clinical condition e.g.
    • Acute stroke/TIA
    • GI haemorrhage
    • Chest pain/acute cardiac problems
    • Suspected DVT or PE
  • Patients with advanced dementia unable to participate in rehabilitation

Patients whose needs can be addressed by community services

  • Memory Clinic patients: Should be referred to the MATS Service (Memory Assessment and Treatment Service in Psychiatry of Old Age)
