The East Lothian Prevention of Admission pathway is hosted within the East Lothian Rehabilitation Service (ELRS) and is delivered by a team of Physiotherapy and Occupational Therapy practitioners based within the community.
The team’s role is to support patients from a therapy perspective who are at risk of admission within the next 24 hours and for whom the GP is happy to manage medical needs at home. A comprehensive clinical assessment will be completed, and any interventions provided, aiming to manage the patient safely at home.
The core hours of the service are Monday – Friday 8am – 4pm.
Currently there is no provision to accept referrals out of hours.
Interventions provided include:
- Transfer assessment and provision of aids
- Mobility assessment and provision of aids
- Assessment of activities of daily living (either independently or with support of carers/family)
- Graded practice of selected activities to improve function
- Onward referral for package of care as indicated
