Self Management Support for LTCs – Edinburgh

This service is suitable for people living in Edinburgh (over the age of 18) with one or more long term health conditions.  The effects of such conditions can be difficult to manage, and people can become quite isolated.

Please consider referrals for anyone who is feeling this way and is interested in accessing support.  The person must be willing to engage with suggestions for change. Please see referral guidance below for details of who is most likely to benefit.

Through person-centred conversations, the Self Management Support Service can help people to:

  • Connect to appropriate community supports and services
  • Access resources to gain a better understanding of their long- term health condition
  • Develop action plans which could result in better health and wellbeing outcomes
  • Identify strategies to assist them to manage their emotional and physical challenges.

The Self-Management Support Worker supports individuals for up to 3 months to help with some, or all, of the above.

This service is provided in partnership between Edinburgh Health and Social Care Partnership’s Long Term Conditions Team and the Lothian Centre for Inclusive Living.

Who to refer:

Anyone can refer to the Self Management Support Service and patients can self-refer if they wish.

  • The individual must be living in Edinburgh and over the age of 18.
  • The individual MUST have a persistent and underlying health condition which is significantly impacting on the quality of their life.
  • The individual MUST be keen for a referral to the service to be made and be willing to engage with the suggestions that are made for there to be a benefit. 
  • The individual is likely to be isolated (might live on their own) or have few social networks.
  • The individual might well appear to have a poor understanding of their long- term health condition and how it impacts on them day to day.
  • The individual is likely to be suffering from a combination of low mood, self-esteem issues, fatigue, anxiety and may even appear to be falling into crises.
  • The individual may well not have engaged with many services in the past but now be keen to do so.

How to refer:

Complete the Edinburgh LTC Self Management Referral Form and email the completed form to ci.loth.smw@nhs.scot

Patients can also self-refer by emailing the following details (no referral form needed for self-referral):
