Hospital at Home is a service that provides acute, hospital-level care by healthcare professionals in a home context for a condition that would otherwise require acute hospital inpatient care.

The H@H referral and exclusion criteria is as follows:
Referral Criteria
- Resident of West Lothian
- Patient/Carer/Family agree to referral
- Patient would otherwise require admission to hospital
- Safe for assessment at clinic or at home
Exclusion Criteria
- Acute stroke
- Suspected fracture
- Surgical problems
- Cardiac chest pain
- Social Issues Only
- Acute Asthma
- Suspected DVT/PTE

Please do not send a SCI Gateway referral unless the referral has already been accepted as the team may not see it and the referral will not be acted on.
The Single Point of Care (SPoC) team will also take referrals. This is for people who need urgent access to community health and/or social care teams(s) to continue to be safely managed at home. For more details see Single Point of Contact (SPoC)
