SW)&nbsp;<\/li>\n\n\n\n<li>2 referral slots a day <em>per locality hub <\/em>are&nbsp;available from <strong>Monday to Friday<\/strong>. <\/li>\n\n\n\n<li><strong>Referral Times are 8.30am \u2013 2pm. <\/strong><\/li>\n<\/ul>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<h4 class=\"wp-block-heading\">Who to refer:<\/h4>\n\n\n\n<p>Rapid response required to support patients who present with:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Inability to transfer\/ mobilise to toilet or attend to personal care<\/li>\n\n\n\n<li>Inability to access food and drink<\/li>\n\n\n\n<li>Inability to self-manage medication<\/li>\n\n\n\n<li>Rapid decline in cognition or social situation resulting in vulnerability in current home.<\/li>\n\n\n\n<li>Resident in Edinburgh<\/li>\n<\/ul>\n\n\n\n<p>The above criteria may also include a response to <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/edinburghfalls\/\" data-type=\"URL\" data-id=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/edinburghfalls\/\" target=\"_blank\" rel=\"noreferrer noopener\">falls&nbsp;<\/a>that present in a deterioration in function as described above<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Who not to refer:<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Prevention of admissions that could be responded to the next day.\u00a0For these referrals please use existing Social Care Direct referral route.<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<h4 class=\"wp-block-heading\">How to refer:<\/h4>\n\n\n\n<p>Call the Flow Centre on 03000 13 4000 Option 1 &amp;
