This provides detail on the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Clinical features at presentation<\/li>\n\n\n\n<li>Risk factors for prostate cancer<\/li>\n\n\n\n<li>Prostate examination in transgender patients<\/li>\n\n\n\n<li>PSA testing and referral in older patients and those living with frailty<\/li>\n\n\n\n<li>PSA testing in asymptomatic patients<\/li>\n\n\n\n<li>How to avoid false positives when performing a PSA test<\/li>\n<\/ul>\n\n\n\n<p class=\"has-standard-black-color has-text-color has-link-color wp-elements-7e97895aef8fc1a6d9f6f1019a50327c wp-block-paragraph\"><strong>D.M. &amp; H.S 19-02-26<\/strong><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<h4 class=\"wp-block-heading\">Who to refer:<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>The following patients should be referred as <mark style=\"background-color:#ffffff\" class=\"has-inline-color has-alert-red-color\">Urgent Suspicion of Cancer<\/mark>:<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Patients with raised PSA according to the following age-adjusted limits:<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Under 70 \u2265 3ug\/L<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">70-79 \u2265 5ug\/L<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">80 and above \u2265 20ug\/L<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For men 80-85 who are fit and do not have multiple co-morbidities if PSA \u226510 but <mark style=\"background-color:#ffffff\" class=\"has-inline-color has-alert-red-color\">&lt;20 refer to Urology as Urgent<\/mark><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">(Be aware that 5\u03b1-Reductase Inhibitors can reduce PSA by up to 50% after 3 months and results should be adjusted accordingly)<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Patients with a Digital Rectal Examination (DRE) suspicious of prostate cancer (hard and\/or irregular) with any PSA.<\/strong><\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Who not to refer:<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Men who have a raised PSA that is suspected to be spurious.\n<ul class=\"wp-block-list\">\n<li>If benign cause suspected and normal DRE &#8211; repeat PSA in 6 weeks<br>(If ejaculation or vigorous exercise within 48hours repeat within 2 weeks)<\/li>\n\n\n\n<li>If remains raised&nbsp; <strong><mark style=\"background-color:#ffffff\" class=\"has-inline-color has-alert-red-color\">Refer to Urology as Urgent Suspected Cancer<\/mark><\/strong><\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Please use the principles of Realistic Medicine when testing and referring elderly and those with significant co-morbidities.<\/li>\n\n\n\n<li>Men with significant co-morbidities, especially those aged 80 or over, should not undergo investigations for prostate cancer without careful consideration of whether a diagnosis of prostate cancer is in their interests.
