S.F. 09-02-24<\/strong><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<h4 class=\"wp-block-heading\">Who to refer:\u200b<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">The Physio @ Home Service is for patients who cannot be seen in any other setting.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patients must fulfil one or more of the following criteria to be eligible for a home visit<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Housebound<\/li>\n\n\n\n<li>Unable to attend clinic by any method<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Other extenuating circumstances, as discussed between the referrer and the service<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Who not to Refer:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Exclusion Criteria<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Physio@Home Service are unable to accept referrals for patients who:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>are currently receiving treatment from physiotherapy in another NHS establishment (e.g. Day Hospital)<em>.<\/em><\/li>\n\n\n\n<li><em><\/em>have received physiotherapy input for the same problem within the last year from any NHS establishment.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">How to&nbsp;refer:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Referrals can be made via SCI Gateway<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">AHP \u2013 Physiotherapy<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Edinburgh \u2013 Astley Ainslie Hospital&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">L Physiotherapy Physio @ Home<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Adult\u00a0<\/strong><a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/P@H-Adult-WA-Self-Referral-Form.docx\" data-type=\"link\" data-id=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/Adult-WA-Self-Referral-Form-22.2.24.docx\" target=\"_blank\" rel=\"noreferrer noopener\">Self-Referral form<\/a><strong>\u00a0for Walking Aid Assessment, Edinburgh<\/strong><\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Primary Care Management<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p class=\"wp-block-paragraph\"><strong>Physiotherapist Prioritisation Criteria<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The following guidelines are for the benefit of Consultants, GPs, and other health professionals referring patients to the Physiotherapy Service.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">All patients should be assessed in person by the referrer and consented for the referral.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is recognised that certain groups of patients are going to need to be seen sooner than others.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Professional judgement based upon reliable information will determine the urgency required in seeking an early physiotherapy appointment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The allocation, however, will be according to&nbsp;clinical judgement and discretion of the physiotherapy service.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patients presenting with one of the following criteria and are at risk of deteriorating are considered to be acute and therefore need urgent appointments:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Recent fractures, joint replacements and recent hospital discharges where early input required<\/li>\n\n\n\n<li>Acute and severe LBP<\/li>\n\n\n\n<li>Acute fall with injury or complex medical history<\/li>\n\n\n\n<li>Acute pain of musculoskeletal nature, if affecting functional activity<\/li>\n\n\n\n<li>Some acute respiratory infections (If the patient presents with COPD please refer to Community Respiratory Team<\/li>\n\n\n\n<li>Patients with complex postural management needs, at risk of deterioration<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">In the event of an acute\/urgent referral, we will endeavour to offer an appointment within 4-6 working weeks (i.e. including weekends), subject to external constraints imposed upon the department concerned, e.g. waiting lists, staffing shortages, etc.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If you want to discuss a referral please contact our Professional Line by phone<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">(North: 07890531695, South: 07890532988).
