{"id":4195,"date":"2022-05-03T11:47:20","date_gmt":"2022-05-03T10:47:20","guid":{"rendered":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/olderpeoplesmentalhealthedinburgh\/"},"modified":"2025-11-06T11:00:42","modified_gmt":"2025-11-06T11:00:42","slug":"olderpeoplesmentalhealthedinburgh","status":"publish","type":"page","link":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/mentalhealthadult\/older-peoples-mental-health\/olderpeoplesmentalhealthedinburgh\/","title":{"rendered":"Older Peoples Mental Health-Edinburgh"},"content":{"rendered":"\n<p><strong>Information <\/strong><\/p>\n\n\n\n<p><strong>Community Mental Health Teams for Older People<\/strong><\/p>\n\n\n\n<p>The Community Mental Health Teams for Older People (North and South Edinburgh) provide: <\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Assessment and treatment to anyone over the age of 65 with a mental health problem<\/li>\n\n\n\n<li>Assessment and treatment for those under 65 years presenting with diagnosis of dementia<\/li>\n\n\n\n<li>Support for individual patients within Care Homes (who do not meet the Edinburgh Behavioural Support Service criteria).&nbsp;<br><\/li>\n<\/ul>\n\n\n\n<p>Please see \u201c<strong>Patients under 65 with cognitive concerns\u201d <\/strong>section below for referral advice if patient is under 65 with clinical suspicion of a neurocognitive disorder and no confirmed diagnosis of dementia<\/p>\n\n\n\n<p>The teams consist of a mix of Community Mental Health Nurses (CMHNs), Community Dementia Nurses (for those aged &lt;65 with dementia), Occupational Therapists, Clinical Psychologists &amp; Older People&#8217;s Psychiatrists.<br><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>North<\/strong> is aligned to the Health &amp; Social Care Partnership NW &amp; NE locality GP boundaries <\/li>\n\n\n\n<li><strong>South<\/strong> is aligned to the Health &amp; Social Care Partnership SW &amp; SE locality GP boundaries <\/li>\n<\/ul>\n\n\n\n<p>Please see <a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/HSCP-Boundaries-Older-Peoples-Mental-Health-Edinburgh.pdf\" data-type=\"URL\" data-id=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/HSCP-Boundaries-Older-Peoples-Mental-Health-Edinburgh.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">HSCP Boundaries Older People&#8217;s Mental Health Edinburgh.pdf<\/a>&nbsp; for boundary definitions. <br><\/p>\n\n\n\n<p><strong>Memory Assessment and Treatment Service<\/strong><\/p>\n\n\n\n<p>MATS assesses patients aged 65+ who present with cognitive decline. The aim is for early diagnosis and intervention. The team consists of CMHNs, occupational therapists, psychiatrists and neurologists. Where assessment confirms a diagnosis of dementia, MATS serves to initiate and monitor newly commenced relevant medication and other relevant interventions. Onward referrals and signposting seek to ensure post-diagnostic support is facilitated; this is most often via an&nbsp;<a href=\"https:\/\/eur01.safelinks.protection.outlook.com\/?url=https%3A%2F%2Fwww.alzscot.org%2Fliving-with-dementia%2Fgetting-support%2Ffind-support-near-you%2Fdementia-link-worker-edinburgh&amp;data=05%7C02%7Cfrances.creasy3%40nhs.scot%7C5cfd1701f26f4c9e1e7708dd44425c90%7C10efe0bda0304bca809cb5e6745e499a%7C0%7C0%7C638741773996219053%7CUnknown%7CTWFpbGZsb3d8eyJFbXB0eU1hcGkiOnRydWUsIlYiOiIwLjAuMDAwMCIsIlAiOiJXaW4zMiIsIkFOIjoiTWFpbCIsIldUIjoyfQ%3D%3D%7C0%7C%7C%7C&amp;sdata=wHa4xPo1E0hmKwA%2B0uXRoxL0safjpinXXg6uqfyUeAA%3D&amp;reserved=0\" target=\"_blank\" rel=\"noreferrer noopener\">Alzheimer Scotland Link Worker<\/a>.<\/p>\n\n\n\n<p>Please note that MATS requires patients to be physically fit enough to be able to attend the clinic in person alongside a person who can provide collateral history for the patient.<\/p>\n\n\n\n<p><strong>Edinburgh Behavioural Support Service<\/strong><\/p>\n\n\n\n<p>This team works with people living in a care home with dementia and experiencing stress and distress.&nbsp; A psychological approach to behaviour management (The Newcastle Model) is used. <\/p>\n\n\n\n<p><strong>Rapid Response Team<\/strong><\/p>\n\n\n\n<p>The&nbsp;<a href=\"\/refhelp\/guidelines\/RapidResponseTeamEdinburgh\" target=\"_blank\" rel=\"noreferrer noopener\">RapidResponseTeamEdinburgh.aspx<\/a>will see those facing a mental health crisis at risk of admission.<br><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p>Referrals are categorised at triage using the following priorities based around imminent or serious risk to the patient or others (e.g. self harm, violence \/ aggression, challenging behaviour, neglect):<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Emergency<\/strong>&nbsp;\u2013 24hr response from receipt of referral. This is facilitated by the RRT \u2013 please see separate&nbsp;<a href=\"\/refhelp\/guidelines\/RapidResponseTeamEdinburgh\" target=\"_blank\" rel=\"noreferrer noopener\">RRT<\/a>&nbsp;guidance (telephone discussion required with RRT)<\/li>\n\n\n\n<li><strong>Urgent<\/strong>&nbsp;\u2013&nbsp;&nbsp; response within 5 working days (CMHTOP is a Monday &#8211; Friday service) \u2013 telephone discussion needed with the relevant Single Point of Referral<\/li>\n\n\n\n<li><strong>Routine<\/strong>&nbsp;\u2013 at least \/ minimum 4 weeks (2 weeks for EBSS)<\/li>\n<\/ul>\n\n\n\n<p>The aim of the triage process is to obtain sufficient information from the person making the referral to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Determine whether the person requires a mental health service intervention;<\/li>\n\n\n\n<li>Direct each referred person to the team likely to meet the identified need for further assessment or treatment;<\/li>\n\n\n\n<li>Determine the level of risk or distress;<\/li>\n\n\n\n<li>Initiate emergency response where the need is identified;<\/li>\n\n\n\n<li>Identify the service most likely to meet the needs of the person, when a mental health service intervention is not required;<\/li>\n\n\n\n<li>Identify local community services and other relevant services;<\/li>\n\n\n\n<li>Give the referrer clear and concise information about the services available and options for further assessment or treatment;<\/li>\n<\/ul>\n\n\n\n<p>Referrers should note that patients are triaged to a range of appropriate professionals depending on their needs. Referrers can request that the patient be seen by a particular discipline, but this request may not be fulfilled. Patients should be aware of this when being referred.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Who can refer:<\/h4>\n\n\n\n<p><strong>Referrals are accepted from:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>GP clinical teams<\/li>\n<\/ul>\n\n\n\n<p><strong>And specialist services too:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Rapid Response Team (RRT).<\/li>\n\n\n\n<li>Hospital clinicians<\/li>\n\n\n\n<li>REAS Older People Mental Health Inpatient Wards<\/li>\n\n\n\n<li>Adult Mental Health Consultants<\/li>\n\n\n\n<li>Mental Health Assessment Service (MHAS)<\/li>\n\n\n\n<li>Cognitive Disorders Clinic at Anne Rowling Clinic (to facilitate CMHTOP follow up for early onset dementia).<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Who to refer:<\/h4>\n\n\n\n<p>All patients referred to the CMHT must be registered (or due to register) with a GP within the Edinburgh boundary and appropriate to the over-65 service (except in the case of early onset dementia). They should have an organic or functional mental health condition with at least one of the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Require assessment of mental state and diagnosis<\/li>\n\n\n\n<li>History of frequent relapses or hospital admissions<\/li>\n\n\n\n<li>Acute disruptive, disturbing or threatening behaviours to self or others (related to mental state)<\/li>\n\n\n\n<li>At serious risk of neglect, or in a position of vulnerability (due to acute mental health problems), which social care are unable to support<\/li>\n\n\n\n<li>Require advice and management of pharmacological treatment<\/li>\n\n\n\n<li>Requiring assessment for diagnosis of dementia \u2013 <em>please ensure that recent blood screening results &amp; relevant brief cognitive testing scores are included in the referral &#8211; see <\/em><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/mentalhealthadult\/cognitivedifficultiesadult\/dementia\/\" target=\"_blank\" rel=\"noreferrer noopener\"><em>RefHelp dementia page<\/em><\/a><em> for guidance<\/em><\/li>\n\n\n\n<li>Require ongoing support due to severity of mental health condition<\/li>\n\n\n\n<li>Require other specialist treatments such as non-pharmacological treatment including psychological therapies<\/li>\n\n\n\n<li>Capacity assessments when the presentation is complex, and the GP requires a psychiatric opinion&nbsp;\u2013 see the <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/mentalhealthadult\/older-peoples-mental-health\/capacity-assessment\/\" target=\"_blank\" rel=\"noreferrer noopener\">Capacity Assessment page<\/a> for more information<\/li>\n\n\n\n<li>Require follow-up after discharge from hospital regarding above<\/li>\n<\/ul>\n\n\n\n<p><strong><em>Please note that referrals as an alternative to hospital admission would require input from the <\/em><\/strong><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/mentalhealthadult\/older-peoples-mental-health\/olderpeoplesmentalhealthedinburgh\/rapidresponseteamedinburgh\/\"><strong><em>RRT<\/em><\/strong><\/a><strong><em>. &nbsp;<\/em><\/strong><\/p>\n\n\n\n<p><strong><em>Please consider and investigate for delirium if changes in mental state and or behaviour are acute in onset prior to referral, and see the <\/em><\/strong><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/medicineoftheelderlygeriatrics\/delirium\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><em>delirium page<\/em><\/strong><\/a><strong><em> for further guidance.<\/em><\/strong><\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Who not to refer:<\/h4>\n\n\n\n<p><strong>Patients under 65 with cognitive concerns<\/strong><\/p>\n\n\n\n<p>People under 65 years of age with cognitive impairment should be referred to general adult community psychiatric services (CMHT) to assess for reversible psychiatric causes of cognitive impairment such as anxiety and depression.<\/p>\n\n\n\n<p>Patients under 65 years old with cognitive impairment who have been reviewed by General Adult psychiatry and have had depression and or anxiety symptoms optimised should be referred by the specialist to neurology (those with a high suspicion of neurodegenerative dementia may be seen at the Cognitive Disorders Clinic at the Anne Rowling Clinic). &nbsp;<\/p>\n\n\n\n<p>Patients under 65 years old with cognitive impairment with a strong family history of heritable neurocognitive disorders, and or with co-morbid neurological symptoms please refer to Neurology. <strong><\/strong><\/p>\n\n\n\n<h4 class=\"wp-block-heading\">How to refer:<\/h4>\n\n\n\n<p>&nbsp;<strong>Single Point of Referral<\/strong><\/p>\n\n\n\n<p>There is a Single Point of Referral for the North &amp; the South for referrals to the CMHTOP, MATS &amp; EBSS (see below regarding referrals to RRT).<\/p>\n\n\n\n<p><strong><u>GPs should refer using SCI Gateway<\/u><\/strong><strong>:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Edinburgh &#8211;
