{"id":4171,"date":"2022-05-03T11:47:20","date_gmt":"2022-05-03T10:47:20","guid":{"rendered":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/neurorehabilitation\/"},"modified":"2024-12-27T14:34:44","modified_gmt":"2024-12-27T14:34:44","slug":"neurorehabilitation","status":"publish","type":"page","link":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/rehabilitation\/neurorehabilitation\/","title":{"rendered":"Neuro Rehabilitation"},"content":{"rendered":"\n<p><strong>Information<\/strong><\/p>\n\n\n\n<p>Rehabilitation medicine specialists provide expertise in addressing mutually aggravating impairments that occur in neurological disorders.<\/p>\n\n\n\n<p>Impairments of speech; mobility; dexterity; cognition; continence; swallow; emotional regulation; &amp; sexuality can all impact on each other, and co-existing complications such as skin breaks; pain; spasticity; nausea; deconditioning or mood disorder can further aggravate or be aggravated by these impairments.<\/p>\n\n\n\n<p>Rehabilitation physicians use their expertise &amp; skills and work with other specialist colleagues and multidisciplinary teams to bring a coordinated holistic approach to enable clients to maximise adjustment, compensatory strategies and autonomy.<\/p>\n\n\n\n<p><strong>M.A. &amp; Y.J. 28-11-24<\/strong><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p>The service is predominantly targeted at patients of working age, but this does not exclude referring older clients who have or realistically aspire to having active social or vocational roles<\/p>\n\n\n\n<p>You may wish to refer patients<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Whose impairment or symptom patterns persists in a way that seems difficult to manage, understand or accept<\/li>\n\n\n\n<li>Have noted new manifestation or deterioration in their condition<\/li>\n\n\n\n<li>Are challenged by uncertainty about the future<\/li>\n<\/ul>\n\n\n\n<p>Typical conditions managed<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Brain injury of any type (traumatic; anoxic; inflammatory or haemorrhagic). This includes stroke or clients with concussion.<\/li>\n\n\n\n<li>The <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/neurology\/transientischaemicattacktiaandstroke\/\" target=\"_blank\" rel=\"noreferrer noopener\">Life after Stroke clinic<\/a>\u2019 is suitable for people who have ongoing problems post-stroke or TIA e.g. fatigue, emotional distress, cognitive impairment, of for new problems that develop over time e.g. neuropathic pain, or where the diagnosis is uncertain and requires review.<\/li>\n\n\n\n<li>Progressively deteriorating \/ degenerative neurological conditions (e.g. MS; MND; muscular dystrophies) \u2013 depending of level of impairment these clients may be more appropriately managed by community neurological rehabilitation service &nbsp;(needs link) or by services for clients with <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/neurology\/parkinsons\/\" target=\"_blank\" rel=\"noreferrer noopener\">Parkinson\u2019s disease<\/a><\/li>\n\n\n\n<li>Longer term consequences of neurological tumour or cancer treatments<\/li>\n\n\n\n<li>Spinal disorders or peripheral neuropathies (traumatic spinal cord injury clients may be more appropriately referred to <a href=\"https:\/\/www.spinalunit.scot.nhs.uk\/\" target=\"_blank\" rel=\"noreferrer noopener\">national spinal injuries unit<\/a><\/li>\n\n\n\n<li>Functional neurological disorders in which a diagnosis has already been confirmed with neurologist \u2013 you or the client may want to consult <a href=\"https:\/\/neurosymptoms.org\/en\/\" target=\"_blank\" rel=\"noreferrer noopener\">Neurosymptoms.org website<\/a><\/li>\n\n\n\n<li>If impairments specifically relate to spasticity you should refer to <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/rehabilitation\/neurorehabilitation\/spasticity\/\" target=\"_blank\" rel=\"noreferrer noopener\">Spasticity management<\/a><\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Who can refer:<\/strong><\/h4>\n\n\n\n<p>Referrals can be made by GP\u2019s; consultants; nurse practitioners\/specialists working with neurological client groups; or allied health practitioners.<em><\/em><\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Who not to refer:<\/strong><\/h4>\n\n\n\n<p>Clients whose conditions are such that input from one single therapy discipline would suffice should be referred to those disciplines<\/p>\n\n\n\n<p>e.g.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Options for clients whose impairments are limited to mobility or dexterity deficits include\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/services.nhslothian.scot\/ecps\/edinburgh-community-stroke-service\/\" target=\"_blank\" rel=\"noreferrer noopener\">Edinburgh Community Stroke Service<\/a><\/li>\n\n\n\n<li><a href=\"https:\/\/policyonline.nhslothian.scot\/wp-content\/uploads\/2024\/04\/Neuro-Rehabilitation-Service.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Neurorehabilitation Occupational Therapy<\/a><\/li>\n\n\n\n<li>Neurological Rehabilitation Therapies \u2013 <a href=\"mailto:loth.neurorehaboutpatientservice@nhs.scot\" target=\"_blank\" rel=\"noreferrer noopener\">loth.neurorehaboutpatientservice@nhs.scot<\/a><\/li>\n\n\n\n<li><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/rehabilitation\/east-lothian-rehabilitation-service-elrs\/east-lothian-community-physiotherapy\/\" target=\"_blank\" rel=\"noreferrer noopener\">East Lothian Rehabilitation Service<\/a><\/li>\n\n\n\n<li><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/rehabilitation\/neurorehabilitation\/crabis-west-lothian\/\" target=\"_blank\" rel=\"noreferrer noopener\">Community Rehabilitation and Brain Injury Service<\/a><\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Clients with isolated impairments of speech should be referred to <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/speechandlanguagetherapy\/\" target=\"_blank\" rel=\"noreferrer noopener\">speech &amp; language therapy<\/a><\/li>\n\n\n\n<li>clients who are independent in day to day functioning but who are struggling to return to employment might be better referred to <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/rehabilitation\/lothian-work-support-services-lwss\/\" target=\"_blank\" rel=\"noreferrer noopener\">vocational rehabilitation<\/a><\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>How to refer:<\/strong><\/h4>\n\n\n\n<p>Refer via&nbsp;<strong>SCI Gateway<\/strong>&nbsp;(AAH &gt;
