There is poor correlation between radiological \u2018age-related\u2019 changes and neck pain. Doing a plain cervical x-ray on someone with neck pain and telling them they have cervical spondylosis\/arthritis\/\u201dwear and tear\u201d may be harmful \/reduce the effectiveness of physio. Plain X-ray only indicated if red flags.<br><br><strong>\u201cI need an MR Cervical Spine to manage this patient\u201d<\/strong><br>We often are asked for MR Cervical Spine to \u2018show the cause\u2019 of someone\u2019s pain or because a physiotherapist has suggested it. MR C-spine is rarely helpful:<br><strong>a)<\/strong>&nbsp;because usually it is muscular pain with radiation not radiculopathy;<br><strong>b)<\/strong>&nbsp;cervical radiculopathy usually improves with conservative treatment;<br><strong>c)<\/strong>&nbsp;neurosurgery is rarely offered\/has little evidence base in this area. MR C-Spine is not available open access in NHS Lothian . We request it mainly to look for spinal cord pathology.<br><br><strong>\u201cNerve conduction studies may be helpful\u201d<br><\/strong>Usually neck and arm pain is not radicular. Conservative treatment with <strong>physiotherapy<\/strong> <strong>referral<\/strong> and <strong>analgesia<\/strong>. &nbsp; Consider referral to <strong>Musculoskeletal Services<\/strong> or <strong>Chronic Pain Services<\/strong>. &nbsp; Sensory symptoms mainly in the hand and forearm often dont conform to textbook distributions are usually Carpal Tunnel Syndrome and\/or Ulnar nerve irritation. &nbsp; \u201c<strong>NHS choices \u2013Neck Pain<\/strong>\u201d has useful links to neck exercise videos<\/td><\/tr><tr><td><strong>NECK AND ARM PAIN<\/strong> <strong>&nbsp;<\/strong> <strong>&nbsp;<\/strong> <strong>NO DEFINITE NEUROLOGICAL SIGNS<\/strong><\/td><td><strong>RARELY INDICATED<\/strong><\/td><td><strong>Muscular pain<\/strong> <strong>Cervical Radiculopathy<\/strong> (rarely) &nbsp;<\/td><\/tr><tr><td><strong>NECK AND ARM PAIN NEUROLOGICAL SYMPTOMS AND SIGNS<\/strong><\/td><td><strong>MAY OFTEN BE AVOIDED<\/strong><\/td><td><strong>Muscular pain<\/strong> combined with \u2022 Carpal Tunnel Syndrome \u2022 Ulnar Nerve Irritation \u2022 Non specific sensory disturbance &nbsp; <strong>Cervical Radiculopathy<\/strong> (rarely) <strong>Other causes<\/strong> rare\u2013 e.g. brachial neuritis<\/td><\/tr><tr><td rowspan=\"3\">HAND PARAESTHESIA WITH OR WITHOUT PAIN<\/td><td rowspan=\"3\">MAY OFTEN BE AVOIDED<\/td><td><strong>Carpal Tunnel Syndrome<\/strong> (often whole hand tingling)<\/td><td>Trial of <strong>wrist splints<\/strong> at night for 3 months <strong>prior<\/strong> to <strong>Hand Clinic<\/strong> referral.<\/td><\/tr><tr><td><strong>Ulnar Nerve Symptoms<\/strong> (4th and 5th fingers)<\/td><td>Advise avoid pressure\/prolonged flexion elbow. <strong>Rarely requires referral to Neurology, investigation or treatment <\/strong>\u2013see neurodiagnosis.org<\/td><\/tr><tr><td><strong>Other<\/strong><\/td><td>Consider Neurology referral.<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p class=\"has-alert-red-color has-text-color has-link-color wp-elements-c4617d5ddb79b730734bb832fe3f81b9\"><strong>RED FLAGS include:<\/strong><\/p>\n\n\n\n<p>Systemic upset (weight loss, night sweats, fevers); Signs of spinal cord compression (e.g. gait disturbance, clumsy or weak hands, loss of sexual, Lhermitte&#8217;s phenomenon, bladder or bowel dysfunction); Significant preceding trauma or neck surgery;
