{"id":4149,"date":"2022-05-03T11:47:20","date_gmt":"2022-05-03T10:47:20","guid":{"rendered":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/moeeastlothian\/"},"modified":"2026-07-09T10:57:47","modified_gmt":"2026-07-09T09:57:47","slug":"moeeastlothian","status":"publish","type":"page","link":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/moeeastlothian\/","title":{"rendered":"MoE ELCH"},"content":{"rendered":"\n<h4 class=\"wp-block-heading\">\u200bInformation <\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">For patients with frailty, dementia or delirium, avoidable admission to hospital can be detrimental to both physical and mental health.&nbsp; Patients lose 10% of their body mass per week on bed rest or with minimal activity as is common in hospital.&nbsp; In addition, change of environment can worsen cognitive and behavioural issues in those with dementia or delirium.&nbsp; For some patient\u2019s admission to hospital is still the most appropriate course of action but this document outlines alternatives sources of support and treatment available to prevent this where possible.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Frailty Assessment<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Across Lothian, it is recommended you use the&nbsp;<a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/MoE-HatH-Rockwood-Clinical-Frailty-Scale-April-2020.pdf\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Rockwood Clinical Frailty Scale<\/strong><\/a>&nbsp;in all Medicine of the Elderly referrals and your clinical practice.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>JH &amp; JB 26\/6\/26<\/strong><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p class=\"wp-block-paragraph\">Please see this PDF on&nbsp;<a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/ELCH-REFERAL-PATHWAYS-JUNE-26.pdf\" data-type=\"link\" data-id=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/ELCH-REFERAL-PATHWAYS-JUNE-26.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">all referral pathways for MOE patients in East Lothian<\/a><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Where referral forms are mentioned these can be found under the&nbsp;Resources and Links&nbsp;section of this page.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Who to refer:<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Falls, frailty syndromes including dizziness<\/li>\n\n\n\n<li>Weight loss in frail older patients i.e. clinical frailty score &gt;4<\/li>\n\n\n\n<li>Multiple\/complex symptoms in frail older patients<\/li>\n\n\n\n<li>Single symptom suggestive of malignancy if frail (consider if single organ specialty referral more appropriate especially if clinical frailty score &lt;4)<\/li>\n\n\n\n<li>Possible Parkinson\u2019s disease (please state this in the referral)<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><mark style=\"background-color:#ffffff\" class=\"has-inline-color has-alert-red-color\">UPDATE JUNE 2026 RE MOE-LED IV THERAPIES (blood transfusions and IV therapy): These services are not currently able to be offered at ELCH. East Lothian patients requiring this service should be escalated to RIE if required<\/mark><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">(Please note this will not affect those already having treatment delivered from Haematology in the IV therapies satellite unit, or those where the treatment is arranged via their GI consultant directly with endoscopy)<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For MOE Advice and OPD in East Lothian the pathways are:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">FOR ADVICE ONLY: East Lothian Community Hospital &gt;&gt; Geriatric Medicine &gt;&gt; Geriatric Medicine Advice<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">FOR OUTPATIENT APPPOINTMENT: East Lothian Community Hospital &gt;&gt; Geriatric Medicine &gt;&gt; Basic Sign Referral<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Who not to refer:<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Iron deficiency anaemia (single issue) \u2013 should be referred to GI<\/li>\n\n\n\n<li>Memory loss only \u2013 should be referred to East Lothian Older People\u2019s Mental Health Team (SCI Gateway: Mental Health &gt;&gt; East Lothian \u2013 Psychiatry of Old Age &gt;&gt; LI Psychiatry of Old Age EL)<\/li>\n\n\n\n<li>Persistent vertigo with suspicion of BPPV or vestibular neuritis, consider if patient could be managed via the physiotherapy vestibular pathway: <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/east-lothian-vestibular-pathway\/\" target=\"_blank\" rel=\"noreferrer noopener\">East Lothian Vestibular Pathway<\/a><\/li>\n\n\n\n<li>Incontinence \u2013 consider <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/urology\/bladder-and-bowel-nurse-team\/\">Continence team<\/a> \/ <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/district-nursing-services\/\" target=\"_blank\" rel=\"noreferrer noopener\">DN referral<\/a><\/li>\n\n\n\n<li>End of life support \u2013 please see the <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/palliativecare\/st-columbas-hospice\/\" target=\"_blank\" rel=\"noreferrer noopener\">Palliative Care team page<\/a> for details of referral, unless require a Community Bed at ELCH, in which case please see the <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/lothian-hbccc-referrals-elch-ward-1\/\" target=\"_blank\" rel=\"noreferrer noopener\">ELCH Lothian HBCCC referrals (ELCH Ward 1) page.<\/a><\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>ALL the above do not preclude the referral being appropriate \u2013 clinical judgment is required.
