This service will liaise with any of our Intermediate Care Teams to respond to your referral as appropriate. We can provide the following, some within 24hrs if required:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Physiotherapy \u2013 assessment, falls reviews, provide mobility aids, advice, exercises<\/li>\n\n\n\n<li>Occupational therapy \u2013 assessment, falls reviews, provide equipment to aid independence, advice<\/li>\n\n\n\n<li>Care at home \u2013 from 15 minute care visits to prompt client with medication to fully assessed large packages of care to keep clients safe in their own home<\/li>\n\n\n\n<li>Social Work input \u2013 urgent assessments and reviews of clients in crisis at home, to prevent social hospital admissions<\/li>\n\n\n\n<li>Liaising with medical\/nursing teams to prevent hospital admission and deliver treatment at home<\/li>\n\n\n\n<li>A place of safety for clients in crisis, when all options for community support have been exhausted<\/li>\n<\/ul>\n\n\n\n<p>We will be communicating directly with the teams you may have had contact with before but aim to prevent professionals calling multiple teams to access the correct care for your patient.<\/p>\n\n\n\n<p>We have links with all Midlothian Intermediate Care Teams: Rapid Response, Discharge to Assess, Community Respiratory Team, MSK Physiotherapy, Hospital at Home, Homecare, District Nurses, Duty Social Work, Dementia Team, etc.&nbsp;<\/p>\n\n\n\n<p><strong>C.W &amp; D.C 27-04-23<\/strong><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<h4 class=\"wp-block-heading\">\u200bWho to refer:<\/h4>\n\n\n\n<p>Refer patients where rapid response is required to support patients who present with:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Inability to transfer\/ mobilise to toilet or attend to personal care<\/li>\n\n\n\n<li>Inability to access food and drink<\/li>\n\n\n\n<li>Inability to self-manage medication<\/li>\n\n\n\n<li>Rapid decline in cognition or social situation resulting in vulnerability in current home.<\/li>\n\n\n\n<li>Resident in Midlothian<\/li>\n<\/ul>\n\n\n\n<p>If you need any advice on the referral, or are unsure if it is appropriate please call 07827 880014 to discuss in the first instance.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Who not to refer :<\/h4>\n\n\n\n<p><strong>Please note<\/strong>&nbsp;&#8211; &nbsp;We are not a medical service and while we can liaise with all the above teams to coordinate the correct care for Midlothian patients we&nbsp;<strong>do not take urgent referrals for the following services<\/strong>:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Mental Health (contact IHTT until 8am to midnight, and MHAS from midnight until 8am)<\/li>\n\n\n\n<li>Hospital at Home (please use their current referral pathways)<\/li>\n\n\n\n<li><a href=\"\/refhelp\/guidelines\/MidlothianCommunityRespiratoryTeam\" target=\"_blank\" rel=\"noreferrer noopener\">Community Respiratory Team&nbsp;<\/a><\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">How to refer: &nbsp;<\/h4>\n\n\n\n<p>Please send electronic referral via SCIgateway to Midlothian Community Hospital &gt; Midlothian &#8211; SPOA &gt; Midlothian SPOA Referral.<\/p>\n\n\n\n<p>If you do not have access to SCIgateway, please complete and email the referral form below to <a href=\"mailto:MidlothianFlowHub@nhslothian.scot.nhs.uk\">MidlothianFlowHub@nhslothian.scot.nhs.uk<\/a><\/p>\n\n\n\n<p><a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/SPOA-referral-form-JAN25.docx\" data-type=\"link\" data-id=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/SPOA-referral-form-JAN25.docx\" target=\"_blank\" rel=\"noreferrer noopener\">SPOA Referral form 2025<\/a><\/p>\n\n\n\n<p>If you need any advice on the referral, or are unsure if it is appropriate please call the Midlothian SPOA on 07827 880014 to discuss in the first instance.<\/p>\n\n\n\n<p><\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Resources and Links<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p><a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/SPOA-referral-form-JAN25.docx\" data-type=\"link\" data-id=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/SPOA-referral-form-JAN25.docx\" target=\"_blank\" rel=\"noreferrer noopener\">SPOA referral form<\/a><\/p>\n\n\n<\/div><\/div>\n<\/div>\n","protected":false},"excerpt":{"rendered":"<p>Health care professionals requiring community input for Midlothian patient should contact the Midlothian Single Point of Access (part of Midlothian Flow Hub).
