BPPV and Vestibular migraine are much more common differential diagnoses for recurrent episodes of vertigo and should be considered first: please see the <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/entadult\/ear\/entdizzinessandbalance\/\">ENT Dizziness and Balance<\/a> page for more detail.<\/p>\n\n\n\n<p>If the patient has had a single episode of persistent vertigo +\/- unilateral hearing loss with prolonged recovery, Vestibular Neuronitis or Labyrinthitis should be considered.<\/p>\n\n\n\n<p><strong>Meniere\u2019s disease is characterised by recurrent attacks of<\/strong>:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>SEVERE AND DISABLING VERTIGO<\/strong> &#8211; fully incapacitating &#8211; so that the patient cannot stand or walk. Vertigo attacks are usually prolonged (persisting for hours).<\/li>\n\n\n\n<li><strong>SIGNIFICANT unilateral HEARING LOSS <\/strong>\u2013 initially fluctuating and then more profound with progression.<\/li>\n\n\n\n<li><strong>Unilateral TINNITUS<\/strong> \u2013 classically &#8216;roaring&#8217;, initially during attacks and eventually more persistent.<\/li>\n<\/ul>\n\n\n\n<p><strong>Unilateral &#8216;AURAL FULLNESS&#8217; <\/strong>may be experienced by some \u2013 a sensation of pressure in the ear, or ear discomfort, often present in advance of a vertigo attack.<\/p>\n\n\n\n<p>Any of these symptoms may predominate, and attacks can occur in clusters.<\/p>\n\n\n\n<p>NICE has produced a helpful CKS on <a href=\"https:\/\/cks.nice.org.uk\/topics\/menieres-disease\/\">Meniere\u2019s<\/a>. &nbsp;A <a href=\"https:\/\/menieres.org\/menieres-disease-diet\/low-sodium-diet-for-menieres-disease\/\">low salt \/ caffeine diet<\/a> may help and the <a href=\"https:\/\/www.menieres.org.uk\/\">Meniere\u2019s Society<\/a> gives further detail, as well as extensive other information.<\/p>\n\n\n\n<p><strong>C.M. &amp; L.McM. 25-03-25<\/strong><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<h4 class=\"wp-block-heading\">Who to refer:<\/h4>\n\n\n\n<p>Where a diagnosis of Meniere&#8217;s needs to be confirmed \u2013 patients with disabling, prolonged vertigo attacks with associated symptoms (see above).<\/p>\n\n\n\n<p>Please first:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Please assess for BPPV with Dix-Hallpike testing as often both are present<\/li>\n\n\n\n<li>Advise about a low salt and caffeine diet<\/li>\n\n\n\n<li>Ask the patient to complete a symptom diary.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Who not to refer:<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>&nbsp;Those with tinnitus but no other ENT symptoms&nbsp;\u2013 see&nbsp;<a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/tinnitusunilateralorbilateralandhyperacusis\/\" data-type=\"URL\" data-id=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/tinnitusunilateralorbilateralandhyperacusis\/\" target=\"_blank\" rel=\"noreferrer noopener\">Tinnitus page<\/a>&nbsp;for advice about self-management or referral to Audiology.<\/li>\n\n\n\n<li>&nbsp;Those with symptoms suggestive of BPPV&nbsp;\u2013 see&nbsp;<a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/bppvbenignparoxysmalpositionalvertigo\/\" data-type=\"URL\" data-id=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/bppvbenignparoxysmalpositionalvertigo\/\" target=\"_blank\" rel=\"noreferrer noopener\">BPPV<\/a>&nbsp;for details of referral to Audiology.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">How to refer:<\/h4>\n\n\n\n<p>Please refer through SCI Gateway to ENT at LAURISTON ONLY.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Primary Care Management<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p><\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Resources and Links<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p><a href=\"https:\/\/cks.nice.org.uk\/topics\/menieres-disease\/\" target=\"_blank\" rel=\"noreferrer noopener\">Meniere\u2019s disease NICE CKS<\/a><\/p>\n\n\n\n<p><a href=\"https:\/\/www.menieres.org.uk\/\" target=\"_blank\" rel=\"noreferrer noopener\">Meniere\u2019s Society<\/a><\/p>\n<\/div><\/div>\n<\/div>\n","protected":false},"excerpt":{"rendered":"<p>&#8203;Most people referred to ENT with possible Meniere&rsquo;s disease do not have the condition, which is NOT associated with mild or brief episodes of vertigo.
