Please refer any suspected cases to the lipid clinic for advice on management, genetic testing and family cascade screening. Referral is recommended if the answers below are &#8216;yes&#8217; to:(1 and\/or 2)..plus (3 or 4 or 5):<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table><tbody><tr><td>\u200b1. Highest total cholesterol &gt;7.5&#8230;&#8230;&#8230;.<\/td><td>\u200b.yes\/no<\/td><\/tr><tr><td>\u200b2. Highest LDL cholesterol &gt;4.9.<\/td><td>\u200b.yes\/no<\/td><\/tr><tr><td>\u200b3. Total cholesterol &gt;7.5 in 1st or 2nd degree relative&#8230;<\/td><td>\u200b.yes\/no<\/td><\/tr><tr><td>\u200b4. Tendon Xanthoma in patient or 1st or 2nd degree relative&#8230;<\/td><td>\u200b.yes\/no<\/td><\/tr><tr><td>\u200b5. MI &lt; 60yrs in 1st or &lt;50yrs in 2nd degree relative<\/td><td>\u200b.yes\/no<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p>Please note that below 16 years the corresponding cut-offs are total cholesterol &gt;6.7 mmol\/L, LDL-C &gt;3.9 mmol\/L. These guidelines should not be used to decide on referral of relatives of known cases of Familial Hypercholesterolaemia and first degree relatives have a 50% pre-test probability of being affected, and lower cholesterol cut-offs apply. If your patient does not meet the above criteria but you still strongly suspect an inherited hyperlipidaemia please contact the lipid clinic via e-mail for further advice.<\/p>\n\n\n\n<p><strong>Other suspected familial hyperlipidaemias<\/strong><\/p>\n\n\n\n<p>Familial combined hyperlipidaemia should be considered in patients with a mixed hyperlipidaemia and strong family history of premature IHD.<\/p>\n\n\n\n<p><strong>Hypertriglyceridaemia<\/strong><\/p>\n\n\n\n<p>Fasting TGs &gt; 10mmol\/L<\/p>\n\n\n\n<p>Fasting TGs persistently 5\u201310mmol\/L in a high cardiovascular risk patient not responding to statin treatment, especially in the absence of a secondary cause such as poorly controlled diabetes or alcohol excess<\/p>\n\n\n\n<p><strong>Lipid Clinics (WGH CVR)<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Advice on lipid lowering therapy, in particular intolerance to&nbsp;&nbsp;\u22653 different statins<\/li>\n\n\n\n<li>Complex cardiovascular risk assessment<\/li>\n\n\n\n<li>Familial hyperlipidaemia\n<ul class=\"wp-block-list\">\n<li>Definite: Total cholesterol &gt;7.5mmol\/L or LDL &gt;4.9 mmol\/L&nbsp;<em>PLUS<\/em>&nbsp;tendon xanthomas in the patient or a first\/second degree relative<\/li>\n\n\n\n<li>Probable: Total cholesterol &gt;7.5mmol\/L or LDL &gt;4.9mmol\/L&nbsp;<em>PLUS<\/em>, one of\n<ul class=\"wp-block-list\">\n<li>Family history of MI: &lt;60 years in a first degree relative or &lt;50 years in a second degree relative<\/li>\n\n\n\n<li>Total cholesterol &gt;7.5mmol\/L in a first\/second degree relative<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Hypertriglyceridaemia<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Who not to refer:<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Advice on the management of patients who, for whatever reason, are unable to tolerate treatment, especially statin therapy.<\/li>\n\n\n\n<li>Specific advice as to whether or not to use drug therapy in a patient with abnormal lipid results.<\/li>\n\n\n\n<li>Advice on the management of patients already under treatment but where there has apparently been a poor response to treatment and\/or target lipid levels have not been achieved.<\/li>\n\n\n\n<li>Advice on the significance of abnormal liver function test results either prior to contemplating starting treatment, or arising during the course of drug therapy.<\/li>\n\n\n\n<li>Advice on the significance of raised CK results again either before initiating treatment or having been found to be elevated during the course of drug treatment.<\/li>\n\n\n\n<li>Questions as to the significance of mildly raised triglyceride levels in patients and how these are best managed.<\/li>\n<\/ul>\n\n\n\n<p><strong>Lipid clinics (WGH CVR)<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Patients on lipid-lowering therapy with sustained total cholesterol reduction &gt;25%<\/li>\n\n\n\n<li>Dyslipidaemia where secondary causes e.g. excess alcohol, uncontrolled diabetes, hypothyroidism, liver disease and nephrotic syndrome has not been excluded\/managed.<\/li>\n\n\n\n<li>Rationalisation (if possible) of drugs associated with dyslipidaemia e.g. thiazide, \u03b2-blockers, retinoids, anti-retrovirals, synthetic oestrogen\/ progesterone<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">How to refer:<\/h4>\n\n\n\n<p>Use SCI Gateway system to choose the required service and location<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Royal Infirmary of Edinburgh<\/li>\n\n\n\n<li>Western General Hospital<\/li>\n\n\n\n<li>St John\u2019s Hospital<\/li>\n<\/ul>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Resources and Links<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p>More information on Lipid and\u00a0Hypertension services is available at the\u00a0University of Edinburgh\u00a0site\u00a0<a href=\"https:\/\/bloodpressureclinic.ed.ac.uk\/home-blood-pressure-monitoring\" target=\"_blank\" rel=\"noreferrer noopener\">Home blood pressure monitoring | Lothian Hypertension and Lipid clinics<\/a> &amp;
