{"id":3994,"date":"2022-05-03T11:47:20","date_gmt":"2022-05-03T10:47:20","guid":{"rendered":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/hearinglosssuddensensorineural\/"},"modified":"2024-12-02T16:43:26","modified_gmt":"2024-12-02T16:43:26","slug":"hearinglosssuddensensorineural","status":"publish","type":"page","link":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/entadult\/ear\/hearinglosssuddensensorineural\/","title":{"rendered":"Hearing Loss (Sudden) Sensorineural"},"content":{"rendered":"\n<p class=\"has-alert-red-color has-text-color has-regular-font-size\" style=\"text-decoration:underline\"><strong>Sudden sensorineural hearing loss is an otological emergency \u2013 please contact ENT on call same day \u2013 see Referral Guidelines below for further detail, <u>including of the \u2018Starfish\u2019 trial<\/u><\/strong><\/p>\n\n\n\n<p><strong>Presentation of Sudden Sensorineural Hearing Loss (SSNHL)<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>\u200bA rapid loss of hearing that occurs suddenly or over a period of up to 72 hours<\/li>\n\n\n\n<li>Typically unilateral<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Mean age of presentation 30-60 years<\/li>\n\n\n\n<li>Estimated incidence in adults of between 5 and 30 cases per 100,000\/year<\/li>\n\n\n\n<li>Sometimes associated with tinnitus\/vertigo.<\/li>\n<\/ul>\n\n\n\n<p><strong><u>Causes<\/u><\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Idiopathic \u2013 commonest cause<\/li>\n\n\n\n<li>Infection \u2013 viral (e.g. mumps, cytomegalovirus, varicella\/zoster), bacterial (syphilis, meningitis)<\/li>\n\n\n\n<li>Autoimmune \u2013 Sarcoidosis, Wegener granulomatosis<\/li>\n\n\n\n<li>Vascular \u2013 Hypercoagulable states<\/li>\n\n\n\n<li>Tumour &#8211; Vestibular Schwannoma, temporal bone metastases<\/li>\n\n\n\n<li>Trauma &#8211; Temporal bone fracture, acoustic trauma, head injury<\/li>\n\n\n\n<li>Drugs &#8211; Aminoglycoside antimicrobials, cisplatin.<\/li>\n<\/ul>\n\n\n\n<p><strong><u>Prognosis <\/u><\/strong><strong><u><\/u><\/strong><\/p>\n\n\n\n<p>Spontaneous recovery from SSNHL varies in studies from 47-63% with the caveat that different studies used different criteria for degrees of recovery.&nbsp; Period of recovery varies from days to weeks.&nbsp; Poor prognostic indicators include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Age: &lt;15 and &gt;65 years<\/li>\n\n\n\n<li>Associated vertigo\/imbalance<\/li>\n\n\n\n<li>High frequency\/profound hearing loss<\/li>\n\n\n\n<li>Bilateral hearing loss.<\/li>\n<\/ul>\n\n\n\n<p><strong><u>STARFISH TRIAL.<\/u><\/strong><\/p>\n\n\n\n<p>ENT is conducting a randomised controlled trial to assess best treatment and it is crucial that patients have not received prior steroids. This is an&nbsp;<strong>otological emergency. <\/strong><\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Who not to refer<\/strong>:<\/h4>\n\n\n\n<p><strong>Patients with no evidence of sensorineural hearing loss on examination, or who clearly have another cause for hearing loss. <\/strong><strong><\/strong><\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>How to refer<\/strong>:<\/h4>\n\n\n\n<p class=\"has-alert-red-color has-text-color has-link-color wp-elements-0c023053afdc5d66e6c96dcb20ccfc38\"><strong>Please refer <\/strong>the same day by&nbsp;<strong>contacting the&nbsp;ENT on Call via St John\u2019s Hospital, Livingston via switchboard.<\/strong><strong><\/strong><\/p>\n\n\n\n<p><strong>Please do not prescribe <\/strong><strong>oral steroids prior to assessment by ENT &#8211; <\/strong>the majority referred do not have sensorineural loss when further assessed and steroids contribute to iatrogenic harm. It is helpful to mention the Starfish trial to patients at the point of referral so that they are aware of what they might be offered when seen.<\/p>\n\n\n\n<p><strong>The on-call ENT clinician will ask<\/strong><strong> on the telephone call for all the following, and patients do need to be physically examined by the referring clinician:<\/strong><\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li><a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/Whisper-test-for-hearing-loss.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Failed whisper test&nbsp;<\/a>\u2013 this is crucial to reliably establish hearing loss<\/li>\n\n\n\n<li>Positive or false negative <a href=\"https:\/\/www.thebsa.org.uk\/wp-content\/uploads\/2023\/10\/OD104-51-BSA-Recommended-Procedure-Rinne-Weber-Tuning-Fork-Tests-February-2022.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Rinne tuning fork test<\/a> \u2013 wherever possible \u2013 to help distinguish conductive vs neurological hearing loss&nbsp;<\/li>\n\n\n\n<li>No other explanation for fluctuation in hearing from clinical history, examination or the medical record<\/li>\n\n\n\n<li>No glue ear \/ wax &lt;90% occlusion of canal.<\/li>\n<\/ol>\n\n\n\n<p><strong>Management by ENT<\/strong><strong><\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Relevant history taking to establish a cause<\/li>\n\n\n\n<li>Otoscopic\/microscopic examination of the ear<\/li>\n\n\n\n<li>Audiometric assessment<\/li>\n\n\n\n<li>MRI\/CT \u2013 inner ear\/brain \u2013 to exclude a retrocochlear cause like acoustic neuroma<\/li>\n\n\n\n<li>Targeted blood studies based on history to ascertain a systemic causes<\/li>\n\n\n\n<li>Follow up audiometry and referral for hearing aid if needed<\/li>\n\n\n\n<li>Decision about treatment including modality with offer of the Starfish RCT <\/li>\n<\/ul>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Primary Care Management<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p>Discuss any case over the phone (contacting the&nbsp;
