The Pathways have beendesigned to support <strong>early identification<\/strong> and the <strong>management of individuals<\/strong> at risk of harm from falls. Using a consistent and structured approach to categorising risk enables equitable access to effective, person-centred support and care.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The <a href=\"https:\/\/staff.nhslothian.scot\/communityfallsprevention\/\" target=\"_blank\" rel=\"noreferrer noopener\">guidance<\/a> is divided into three sections:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">1. <a href=\"https:\/\/staff.nhslothian.scot\/communityfallsprevention\/framework-and-toolkit\/\" target=\"_blank\" rel=\"noreferrer noopener\">Framework and Toolkit<\/a>: This uses a model of risk stratification for falls based on a population health approach, which can be used by a variety of health and care professionals. This tool enables people to be identified and categorised by risk of harm. Following this screening, people are categorised into falls risk levels:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Level 0 &#8211; very low risk<\/li>\n\n\n\n<li>Level 1 &#8211; low risk<\/li>\n\n\n\n<li>Level 2 &#8211; moderate risk<\/li>\n\n\n\n<li>Level 3 &#8211; high risk<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Please see under the Referral Management section for how <a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/MoE-HatH-Rockwood-Clinical-Frailty-Scale-April-2020.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Clinical Frailty Scoring (CFS)<\/a> can be used for falls risk stratification.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">2. <a href=\"https:\/\/staff.nhslothian.scot\/communityfallsprevention\/care-bundles\/\" target=\"_blank\" rel=\"noreferrer noopener\">Falls Care Bundles<\/a>: Once an individual\u2019s level of risk has been identified they can be referred to the appropriate <a href=\"https:\/\/staff.nhslothian.scot\/communityfallsprevention\/care-bundles\/\" target=\"_blank\" rel=\"noreferrer noopener\">Falls Care Bundle\/s <\/a>\u00a0which ensure that people receive a consistent level of support. In this section information on various resources and interventions are grouped under the different levels of Falls Risk. They include links to information on Health and Wellbeing, Physical Activity and Exercise and Environmental supports<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">3. <a href=\"https:\/\/staff.nhslothian.scot\/communityfallsprevention\/knowledge-and-skills-framework\/\" target=\"_blank\" rel=\"noreferrer noopener\">Knowledge and Skills Framework<\/a>: Provides an outline of the knowledge, skills, and learning required by teams and individuals to deliver the pathway consistently.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>KOR &amp; JB &#8211; 23-7-26<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Resources and Links<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p class=\"wp-block-paragraph\">GPs can use <a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/MoE-HatH-Rockwood-Clinical-Frailty-Scale-April-2020.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Clinical Frailty (CFS)<\/a> based stratification to determine a person\u2019s risk of falling and experiencing harm due to a fall and easily identify what information to sign-post patients to and routes of referral.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Referral advice based on CFS \/ Falls Risk<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>CFS 0-1 (Level 0: Very Low Risk)<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Sign post patient to<strong>\u00a0 <\/strong><a href=\"https:\/\/services.nhslothian.scot\/fallssupport\/i-live-in-edinburgh\/\" target=\"_blank\" rel=\"noreferrer noopener\">I Live in Edinburgh \u2013 Falls Support<\/a> \u2013 see section \u201cCare Pathway for Falls Level 0 (Very Low Risk)\u00a0 Signposting to Public Health Information\u201d<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>CFS 2-5 (Level 1 &amp; 2: Low \u2013 Moderate Risk of Avoidable Harm)<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Referral to Edinburgh Leasure \u201cLEVEL 2 \u2013\u00a0Balanced Life\u201d using their online <a href=\"https:\/\/forms.edinburghleisure.co.uk\/edinburghleisure\/form\/ActiveCommunitiesBalancedLifeReferralForm\/formperma\/uymUbdMpg8rsX5niVwWM0RBw0iFGCw7o6JN13POaJzk\" target=\"_blank\" rel=\"noreferrer noopener\">Active Communities Balanced Life Referral Form<\/a>. Please ensure that the patient has consented to the referral and being contacted by Edinburgh Leasure, who will contact patients directly regarding their referral.<\/li>\n\n\n\n<li>Sign post patient to<strong>\u00a0 <\/strong><a href=\"https:\/\/services.nhslothian.scot\/fallssupport\/i-live-in-edinburgh\/\" target=\"_blank\" rel=\"noreferrer noopener\">I Live in Edinburgh \u2013 Falls Support<\/a><strong>\u00a0 &#8211; <\/strong>see section<strong> \u201c<\/strong>Care Pathway for Level 1 (Low Risk of Avoidable Harm) \u2013 Falls Care Bundle 1\u201d and\/or section \u201cCare Pathway for Level 2 (Moderate of Avoidable Harm) \u2013 Falls Care Bundle 2\u201d<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>CFS 6+ (Level 3: High Immediate Risk of Harm)<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Refer the patient for a Multifactorial Falls Assessment (MFA), to <a href=\"https:\/\/services.nhslothian.scot\/ecps\/edinburgh-hub-services\/\" target=\"_blank\" rel=\"noreferrer noopener\">Edinburgh Hub Services<\/a> by calling\u00a0<a href=\"https:\/\/www.edinburgh.gov.uk\/socialcaredirect\" target=\"_blank\" rel=\"noreferrer noopener\">Social Care Direct<\/a> on the\u00a0clinician-only falls\u00a0telephone numbers 0131 200 2338 \/ 4262.<\/li>\n\n\n\n<li>(Practitioners outwith the GP setting who have access to TRAK can <a href=\"https:\/\/staff.nhslothian.scot\/falls\/wp-content\/uploads\/sites\/27\/2024\/02\/EFARSRequestServiceGuide.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Request for Service for Edinburgh Falls Assessment and Rehabilitation Referrals<\/a>,\u00a0 via TRAK using a\u00a0<a href=\"https:\/\/staff.nhslothian.scot\/falls\/wp-content\/uploads\/sites\/27\/2024\/02\/EFARSRequestServiceGuide.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Request for Service<\/a>)<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Information re other services outwith The Community Falls Prevention and Management pathway<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Assistance to Get Up Off the Floor<\/strong><strong><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For patients who are\u00a0uninjured but stuck on the floor, then on this <a href=\"https:\/\/staff.nhslothian.scot\/falls\/edinburgh\/\" target=\"_blank\" rel=\"noreferrer noopener\">page<\/a> you can find the <strong>Edinburgh Fallen Uninjured Person Pathway<\/strong>.\u00a0This service is\u00a0provided by the\u00a0Assistive Technology Enabled Care\u00a0(ATEC 24)\u00a0monitoring and response\u00a0team\u00a0on behalf of the Edinburgh Health and Social Care Partnership.\u00a0<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Risk of Admission&nbsp;<\/strong><strong><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If the patient is at imminent risk of hospital admission&nbsp;due to falls&nbsp;within the next 4 hours&nbsp;follow the <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/urgenttherapyandsocialcare\/\" target=\"_blank\" rel=\"noreferrer noopener\">Urgent Therapy and Social Care pathway<\/a>&nbsp;by checking the referral criteria. Call&nbsp;the <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/admissionsurgentcare\/flow-navigation-centre\/\" target=\"_blank\" rel=\"noreferrer noopener\">Flow Centre<\/a>&nbsp;8:30am to 2pm&nbsp;on 0300 013 4000&nbsp;option 1 and 4, or <a href=\"https:\/\/www.edinburgh.gov.uk\/socialcaredirect\" target=\"_blank\" rel=\"noreferrer noopener\">Social Care Direct<\/a>&nbsp;on 0131 200 2324&nbsp;outwith these times<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Care Home Residents&nbsp;<\/strong><strong><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Community Falls Prevention and Management pathway is not applicable in Care Homes. If you are working with a&nbsp;care home resident&nbsp;who has had recent falls, worsening or increasing falls, or is at risk of falls,&nbsp;referrals to services that can support the resident with a range of falls risk factors can be made by the care home, the GP, or a range of professionals. to the services here: <a href=\"https:\/\/services.nhslothian.scot\/carehomes\/\" target=\"_blank\" rel=\"noreferrer noopener\">https:\/\/services.nhslothian.scot\/CareHomes\/Pages\/default.asp<\/a><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>MOE Assessment\/Day Hospital&nbsp;<\/strong><strong><\/strong> For patients who would benefit from combined&nbsp;medical, nursing and rehabilitation assessments&nbsp;to reduce their falls risk this can be arranged through referral MOE. In some areas this is to Day Hospital, in other areas this is to MOE for triage as to most appropriate input.
