{"id":3903,"date":"2022-05-03T11:47:20","date_gmt":"2022-05-03T10:47:20","guid":{"rendered":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/edinburghfalls\/"},"modified":"2026-04-16T13:43:43","modified_gmt":"2026-04-16T12:43:43","slug":"edinburghfalls","status":"publish","type":"page","link":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/medicineoftheelderlygeriatrics\/falls\/edinburghfalls\/","title":{"rendered":"Edinburgh Falls"},"content":{"rendered":"\n<p>One in three older people fall every year, and younger people are at risk of falls too.&nbsp;&nbsp;The causes of falls are multiple, and&nbsp;the effects can be significant and wide ranging.&nbsp; However, falls are everyone&#8217;s business, and all of us can help reduce them within our roles.<\/p>\n\n\n\n<p><strong>Assistance to Get Up Off the Floor<\/strong><\/p>\n\n\n\n<p>For patients who are\u00a0uninjured but stuck on the floor, follow the\u00a0<strong><a href=\"https:\/\/staff.nhslothian.scot\/falls\/wp-content\/uploads\/sites\/27\/2024\/02\/EFUPP.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">fallen uninjured person pathway<\/a><\/strong>\u00a0to access urgent support to help them off the floor.\u00a0This service is\u00a0provided by the\u00a0Assistive Technology Enabled Care\u00a0(ATEC 24)\u00a0monitoring and response\u00a0team\u00a0on behalf of the Edinburgh Health and Social Care Partnership.\u00a0<\/p>\n\n\n\n<p><strong>Risk of Admission&nbsp;<\/strong><\/p>\n\n\n\n<p>If the patient is at\u00a0<strong>imminent risk of hospital admission<\/strong>\u00a0<strong>due to falls<\/strong>\u00a0within the next 4 hours\u00a0follow the\u00a0<strong><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/urgenttherapyandsocialcare\/\" target=\"_blank\" rel=\"noreferrer noopener\">Urgent Therapy and Social Care pathway<\/a><\/strong>\u00a0by checking the referral criteria. Call\u00a0the\u00a0<strong><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/admissionsurgentcare\/flow-navigation-centre\/\" target=\"_blank\" rel=\"noreferrer noopener\">Flow Centre<\/a><\/strong>\u00a08:30am to 2pm\u00a0on 0300 013 4000\u00a0option 1 and 4, or\u00a0<strong><a href=\"https:\/\/www.edinburgh.gov.uk\/socialcaredirect\" target=\"_blank\" rel=\"noreferrer noopener\">Social Care Direct<\/a><\/strong>\u00a0on 0131 200 2324\u00a0outwith these times.<\/p>\n\n\n\n<p><strong>Edinburgh Falls Prevention Pathway&nbsp;&nbsp;<\/strong><\/p>\n\n\n\n<p>If&nbsp;you&nbsp;are working with a person of any adult age living in the community who has had recent falls, worsening or increasing falls, or is at risk of falls, you can&nbsp;<strong>support<\/strong>&nbsp;them by following the<strong>&nbsp; <a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/Edinburgh-Falls-Prevention-Pathway-Healthcare-professionals-JAN-2024.pdf\" data-type=\"link\" data-id=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/Edinburgh-Falls-Prevention-Pathway-Healthcare-professionals-JAN-2024.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Edinburgh Falls Prevention Pathway January 2024 for Health Care Professionals<\/a> <\/strong>Pathways for other groups (Social Care &amp; Social Work and Third Sector) can be found under <a><mark style=\"background-color:#ffffff\" class=\"has-inline-color has-standard-black-color\">Resources and Links.<\/mark><\/a><a id=\"_msocom_1\"><\/a><\/p>\n\n\n\n<p><strong>Multifactorial&nbsp;Falls Assessment and Rehabilitation at Home&nbsp;<\/strong><\/p>\n\n\n\n<p>For patients who would benefit from a\u00a0<a href=\"https:\/\/staff.nhslothian.scot\/falls\/edinburgh\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>multifactorial\u00a0falls assessment<\/strong><\/a>\u00a0at home call\u00a0<strong><a href=\"https:\/\/www.edinburgh.gov.uk\/socialcaredirect\" target=\"_blank\" rel=\"noreferrer noopener\">Social Care Direct<\/a><\/strong> &#8211; details of a Clinician only falls Telephone Number can be found on the <strong><a href=\"https:\/\/staff.nhslothian.scot\/falls\/edinburgh\/\" target=\"_blank\" rel=\"noreferrer noopener\">intranet falls page<\/a><\/strong>.\u00a0\u00a0A\u00a0practitioner working within a therapy team will aim to\u00a0visit within 10 days or quicker, depending on urgency.\u00a0\u00a0The practitioner will work with the patients to carry out a multifactorial assessment of both the person and their home\u00a0environment for falls risks. They will advise and support with an individualised\u00a0balance, strength, gait and functional rehabilitation programme. It is an <strong>evidence-based falls prevention programme<\/strong> designed for adults who have had or are at risk of having a fall. In some areas this is to Day Hospital, in other areas this is to MOE for triage as to most appropriate input. See guidance under the relevant <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/medicineoftheelderlygeriatrics\/\" target=\"_blank\" rel=\"noreferrer noopener\">MOE services<\/a>.<\/p>\n\n\n\n<p><strong>Specialist Input to Reduce Falls Risk Factors&nbsp;<\/strong><\/p>\n\n\n\n<p>If the patient requires&nbsp;<strong>specialist input&nbsp;<\/strong>to reduce their identified&nbsp;falls risk&nbsp;factors&nbsp;you can find links to a range of specialist services such as podiatry, dietetics, speech and language therapy, continence services, substance misuse,&nbsp;auditory services, visual services, chronic pain, mental health, community stroke, progressive neurological conditions, occupational therapy or musculoskeletal&nbsp;physiotherapy&nbsp;within Edinburgh on <a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/Edinburgh-Falls-Prevention-Pathway-Healthcare-professionals-JAN-2024.pdf\" data-type=\"link\" data-id=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/Edinburgh-Falls-Prevention-Pathway-Healthcare-professionals-JAN-2024.pdf\" target=\"_blank\" rel=\"noreferrer medication that may be affecting their falls risk a<strong>&nbsp;medication review<\/strong>&nbsp;can be provided by the Primary Care Team (GP, Primary Care Pharmacist).<\/p>\n\n\n\n<p><strong> Vestibular Issues\/Dizziness<\/strong><\/p>\n\n\n\n<p>If your patient has dizziness or balance issues then they can be referred for support:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>For Patients under 65 with a likely ENT cause please follow the referral guidance on <a RefHelp<\/a><\/li>\n<\/ul>\n\n\n\n<p><strong>Community Alarms&nbsp;<\/strong><\/p>\n\n\n\n<p>For patients that might benefit from a\u00a0<strong>community alarm<\/strong>, this allows 24\/7\u00a0support from the\u00a0<strong><a href=\"https:\/\/www.edinburghhsc.scot\/livingindependently\/atec24\/\" target=\"_blank\" rel=\"noreferrer noopener\">ATEC 24\u00a0monitoring and response team<\/a><\/strong>.\u00a0 A referral can be made via\u00a0<strong><a href=\"https:\/\/www.edinburgh.gov.uk\/socialcaredirect\" target=\"_blank\" rel=\"noreferrer noopener\">Social Care Direct<\/a><\/strong>.<\/p>\n\n\n\n<p><strong>Community Equipment&nbsp;<\/strong><\/p>\n\n\n\n<p>For patients who would benefit from\u00a0<strong>assessment\u00a0for equipment to live independently<\/strong>, more support is available.\u00a0 For minor adaptations such as grabrails, handrails and bannisters, a self-referral can be made via the\u00a0<strong><a href=\"https:\/\/www.edinburgh.gov.uk\/orderequipmenthomeform\" target=\"_blank\" rel=\"noreferrer noopener\">online form<\/a><\/strong>.\u00a0 If an\u00a0assessment for other community equipment\u00a0or larger adaptations\u00a0is indicated,\u00a0a referral can be made via\u00a0<strong><a href=\"https:\/\/www.edinburgh.gov.uk\/get-care-support\/ask-social-care-direct-advice-2\/2?documentId=12344&amp;categoryId=20199\" target=\"_blank\" rel=\"noreferrer noopener\">Social Care
