The service is aimed at people with poorly controlled diabetes and those at high risk of hospital admission.<\/p>\n\n\n\n<p>The service is available Monday to Friday 8:00 AM to 6:00 PM (including most public holidays).<\/p>\n\n\n\n<p>Please follow the Edinburgh Community DSN Referral Flow Chart below to ensure the Community Diabetes team is the correct service for your patient.<\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<h4 class=\"wp-block-heading\">Who can refer:<\/h4>\n\n\n\n<p>Referrals are welcome from all Healthcare Professionals and are triaged and prioritised according to need and workload capacity. &nbsp;<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">How to refer:<\/h4>\n\n\n\n<p>Referrals can be submitted via SCI-Gateway &gt; AHP \u2013 Community Diabetes or by completing the <a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/Edinburgh-IMPACT-referral-form.docx\" data-type=\"link\" data-id=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/Edinburgh-IMPACT-referral-form.docx\" target=\"_blank\" rel=\"noreferrer noopener\">DSN referral form<\/a> which should be sent to us via email, along with a patient summary sheet to <a href=\"mailto:loth.community\" target=\"_blank\" rel=\"noreferrer noopener\">loth.community<\/a>diabetesteam@nhs.scot<\/p>\n\n\n\n<p>During acute illness, please contact the on-call diabetes registrar.<\/p>\n\n\n\n<p>If you require one of the following services, please refer to the department directly.
