{"id":3841,"date":"2022-05-03T11:47:20","date_gmt":"2022-05-03T10:47:20","guid":{"rendered":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/depressionadults\/"},"modified":"2026-06-30T14:26:26","modified_gmt":"2026-06-30T13:26:26","slug":"depressionadults","status":"publish","type":"page","link":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/mentalhealthadult\/depressionadults\/","title":{"rendered":"Depression (Adults)"},"content":{"rendered":"\n<h4 class=\"wp-block-heading\">Description<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Depression is a common mental disorder that typically presents with a range of symptoms and varying levels of impairment in social and occupational functioning.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><span style=\"text-decoration: underline\">ICD 10 Diagnostic Criteria<\/span>:<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Depression is defined as the presence of pervasive low mood and a loss of interest and enjoyment in activities, occurring most of the day, nearly every day, for at least 2 weeks, as well as other symptoms such as:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Reduced concentration and attention<\/li>\n\n\n\n<li>Reduced energy or fatigue<\/li>\n\n\n\n<li>Ideas of guilt or unworthiness<\/li>\n\n\n\n<li>Bleak and pessimistic views of the future<\/li>\n\n\n\n<li>Ideas or acts of deliberate self harm or suicide<\/li>\n\n\n\n<li>Disturbed sleep<\/li>\n\n\n\n<li>Significant changes in appetite or weight<\/li>\n\n\n\n<li>Psychomotor agitation or retardation.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Assessment also includes consideration of onset, duration, pattern and severity of symptoms, as well as impact on functioning, risk of self-harm and suicide.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Depressive episodes are subdivided into less severe (encompassing subthreshold and mild depression) and more severe (encompassing moderate and severe depression).<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><span style=\"text-decoration: underline\">Treatment<\/span><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Initial support should include <a href=\"https:\/\/www.nhsinform.scot\/illnesses-and-conditions\/mental-health\/mental-health-self-help-guides\/depression-self-help-guide\" target=\"_blank\" rel=\"noreferrer noopener\">psychoeducation<\/a> <em>(this link can be shared with patients<\/em>) and activities that could improve the person\u2019s sense of wellbeing.&nbsp; Treatment should take a shared decision-making approach and follows a <a href=\"https:\/\/www.nice.org.uk\/guidance\/ng222\/resources\/the-matched-care-model-pdf-11131007011\" target=\"_blank\" rel=\"noreferrer noopener\">matched care model<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Consideration should be given to situational factors such as housing, financial, occupational, social or relationship instability or difficulties that might benefit from an alternative approach through third sector organisations (e.g. Citizen\u2019s Advice Bureau, relationship counselling, Women\u2019s Aid).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><span style=\"text-decoration: underline\">For less severe depressive episodes<\/span>, which can be driven by situational stress, lower intensity options include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Active monitoring<\/li>\n\n\n\n<li><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/mentalhealthadult\/mentalhealthpsychologyotherservices\/computerised-cognitive-behavioural-therapy-cbt\/\" target=\"_blank\" rel=\"noreferrer noopener\">Silvercloud<\/a> (computerised Cognitive Behavioural Therapy, referral via Sci Gateway)<\/li>\n\n\n\n<li><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/mentalhealthadult\/mentalhealthpsychologyotherservices\/physical-activity-for-mental-health\/\" target=\"_blank\" rel=\"noreferrer noopener\">Physical activity for mental health<\/a> referrals<\/li>\n\n\n\n<li>Psychoeducation, including <a href=\"https:\/\/health-in-mind.org.uk\/news\/wellbeing-courses-and-workshops-edinburgh\/#stress\" target=\"_blank\" rel=\"noreferrer noopener\">Stress Control classes<\/a>, where available (self-referral)<\/li>\n\n\n\n<li><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guided-self-help\/\" target=\"_blank\" rel=\"noreferrer noopener\">Guided self-help<\/a> offers a short series of supportive sessions to work through CBT-based self-help materials. These are usually available on a self-referral basis.\n<ul class=\"wp-block-list\">\n<li>In Edinburgh, <a href=\"https:\/\/ithriveedinburgh.org.uk\/self-help\/\" target=\"_blank\" rel=\"noreferrer noopener\">iThrive<\/a> and <a href=\"https:\/\/health-in-mind.org.uk\/news\/wellbeing-courses-and-workshops-edinburgh\/#stress\" target=\"_blank\" rel=\"noreferrer noopener\">Health in Mind<\/a><\/li>\n\n\n\n<li>In West Lothian, the <a href=\"https:\/\/policyonline.nhslothian.scot\/policy_entry\/west_lothian_community_wellbeing_hubs\/\" target=\"_blank\" rel=\"noreferrer noopener\">Wellbeing Hub<\/a><\/li>\n\n\n\n<li>In Midlothian, the <a href=\"https:\/\/health-in-mind.org.uk\/news\/midlothian-access-point\/\" target=\"_blank\" rel=\"noreferrer noopener\">Midlothian Access Point<\/a><\/li>\n\n\n\n<li>In East Lothian, <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/mentalhealthadult\/mentalhealthlocalityservicesadult\/east-lothian-mental-health\/\" target=\"_blank\" rel=\"noreferrer noopener\">CWIC or the Mental Health and Wellbeing Hub<\/a><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">There is more detail on the <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/mentalhealthadult\/mentalhealthlocalityservicesadult\/\" target=\"_blank\" rel=\"noreferrer noopener\">mental health locality page<\/a>.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Antidepressants should not be used initially in mild depression (poor risk benefit ratio). In moderate depression they should be used alongside psychological therapy.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><span style=\"text-decoration: underline\">For more severe or chronic depression, or less severe depression that has not responded to lower intensity interventions above<\/span>, treatment options are ideally both:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Antidepressant treatment \u2013 which should be used alongside psychological therapies. SSRIs remain the first line drug of choice. Please see the <a href=\"https:\/\/formulary.nhs.scot\/east\/central-nervous-system\/depression\/depression\/\" target=\"_blank\" rel=\"noreferrer noopener\">East Lothian Formulary<\/a> for more detail.&nbsp;<\/li>\n\n\n\n<li><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/mentalhealthadult\/mentalhealthpsychologyotherservices\/psychological-therapies-services-pts\/\" target=\"_blank\" rel=\"noreferrer noopener\">Psychological treatment<\/a>, such as Cognitive Behavioural Therapy (CBT, referral via Sci Gateway).<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Patients in crisis, or at high risk of suicide, should be risk assessed and considered for <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/mentalhealthadult\/emergenciesmentalhealthadults\/\" target=\"_blank\" rel=\"noreferrer noopener\">emergency mental health services<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><span style=\"text-decoration: underline\">Antidepressant treatment<\/span><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is generally expected that the GP will have tried two antidepressants and considered psychological approaches, including CBT, before seeking further advice. The Formulary recommendation is to try two different SSRIs initially. <span style=\"text-decoration: underline\">Lithium for treatment and prophylaxis of recurrent major depressive disorder [unipolar depression] should only be initiated by a specialist (BNF advice)<\/span><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><span style=\"text-decoration: underline\">Psychological treatment<\/span><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Information about different psychological therapies and interventions can be found on the <a href=\"https:\/\/www.matrix.nhs.scot\/\" target=\"_blank\" rel=\"noreferrer noopener\">Matrix Guide to Delivering Evidence Based Psychological Therapies and Interventions in Scotland.<\/a>&nbsp; The <a href=\"https:\/\/www.matrix.nhs.scot\/intervention-search\/\" target=\"_blank\" rel=\"noreferrer noopener\">search facility<\/a> for a particular condition or type of patient is helpful.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Information on referral criteria for psychological therapy can be found <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/mentalhealthadult\/mentalhealthpsychologyotherservices\/psychological-therapies-services-pts\/\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a>. Accepted referrals for psychological treatment will usually involve an initial suitability assessment appointment before being added to a treatment waiting list.&nbsp; Some interventions are delivered in group format, but referral for this follows the same pathway and suitability assessment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Psychology and locality mental health services are for adults aged 18-64: please refer to CAMHS or Older People\u2019s Mental Health services for those outwith that age range. The exception is online and specific psychoeducational programmes such as Guided Self Help or cCBT.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>LB &amp; CM 26\/6\/26<\/strong><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<h4 class=\"wp-block-heading\">Who to refer:<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\"><span style=\"text-decoration: underline\">Locality Mental Health Services<\/span><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If a person\u2019s symptoms of depression have not responded to medication or psychological therapy, consider re-visiting other factors (e.g. personal, social or environmental) or adherence to treatment, and discuss options for alternative medication or psychological treatment. This is also an indication for referral. Nice guidelines have a <a href=\"https:\/\/www.nice.org.uk\/guidance\/ng222\/resources\/furtherline-treatment-pdf-11131007009\" target=\"_blank\" rel=\"noreferrer noopener\">helpful infographic<\/a> to aid this.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">NICE CKS summaries are very useful:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/cks.nice.org.uk\/topics\/depression\/management\/initial-management\/\" target=\"_blank\" rel=\"noreferrer noopener\">Initial primary care management<\/a> with a range of patient information sources too<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/cks.nice.org.uk\/topics\/depression\/management\/ongoing-management\/\" target=\"_blank\" rel=\"noreferrer noopener\">Ongoing management<\/a> including 2<sup>nd<\/sup> line therapies and reducing relapse<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/cks.nice.org.uk\/topics\/depression\/prescribing-information\/\" target=\"_blank\" rel=\"noreferrer noopener\">Antidepressant prescribing<\/a><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">An <a href=\"https:\/\/bnf.nice.org.uk\/treatment-summaries\/antidepressant-drugs\/\" target=\"_blank\" rel=\"noreferrer noopener\">overview of antidepressant drug classes<\/a> and their complications.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Who not to refer:<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Stable patients\n<ul class=\"wp-block-list\">\n<li>But note medication advice for stable patients can be requested without need for full referral. This can be done through SCI Gateway by marking clearly on the referral \u2018Advice Only\u2019. This may include individuals receiving Lithium treatment, multiple psychotropic medications or MAOI treatment who are not in outpatient follow up.
