Cough can also result from stimulation of the cough receptors in the lung directly<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<h4 class=\"wp-block-heading\">Who to refer:<\/h4>\n\n\n\n<p>Patients who have<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>identified serious respiratory disease (cancer\/interstitial disease\/severe COPD\/ bronchiectasis)<\/li>\n\n\n\n<li>identified respiratory disease with persistent symptoms despite appropriate treatment (asthma \/COPD)<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Suggested initial management<\/h4>\n\n\n\n<h4 class=\"wp-block-heading\">LRTI&nbsp;&nbsp;&nbsp;<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Amoxicillin 500mg tds for at least one week<\/li>\n\n\n\n<li>Clarithromycin 500mg bd for at least one week<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">COPD\/Asthma<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Bronchial hyper reactivity: trial of prednisolone 40mg for least 5 days<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">GORD&nbsp;<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Appropriate PPI max dose for 3 months +\/-<\/li>\n\n\n\n<li>Domperidone 10mg tds<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Nasal\/Sinus irritation&nbsp;or infection<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Nasal steroids for at least 2 months<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Who not to refer:<\/h4>\n\n\n\n<p>Patients with identified:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>upper airways disease (rhinitis)<\/li>\n\n\n\n<li>GORD.<\/li>\n<\/ul>\n\n\n\n<p>&nbsp;Patients with an established diagnosis with chronic cough being a common symptom :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>COPD(maximise treatment)<\/li>\n\n\n\n<li>interstitial lung disease<\/li>\n\n\n\n<li>bronchiectasis<\/li>\n\n\n\n<li>asthma&nbsp;(maximise treatment )<\/li>\n\n\n\n<li>post-infectious cough (which can last for up to one year).<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">How to refer:<\/h4>\n\n\n\n<p>SCI-Gateway to the appropriate OPD clinic<\/p>\n\n\n\n<p><strong>Investigations to be included with referral:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>chest x-ray<\/li>\n\n\n\n<li>spirometry with reversibility<\/li>\n\n\n\n<li>examination of neck and throat.<\/li>\n<\/ul>\n\n\n\n<p><strong>All diagnoses suspicious of cancer should be marked urgent<\/strong><\/p>\n\n\n\n<p>Note:&nbsp;weight loss,&nbsp;alteration of voice,&nbsp;shortness of breath,&nbsp;haemoptysis &#8211; refer urgently<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Primary Care Management<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p><strong>Common causes are:<br><\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>post-viral cough (can last for a year or longer)<\/li>\n\n\n\n<li>post-infectious cough (airways or sinuses)<\/li>\n\n\n\n<li>an asthma syndrome<\/li>\n\n\n\n<li>rhinitis<\/li>\n\n\n\n<li>gastro oesophageal reflux&nbsp;(GORD)<\/li>\n\n\n\n<li>medication (particularly ACE-inhibitors)<\/li>\n\n\n\n<li>smoking<\/li>\n\n\n\n<li>heart failure<\/li>\n\n\n\n<li>occupational exposure to an irritant or an allergen.<\/li>\n\n\n\n<li><\/li>\n<\/ul>\n\n\n\n<p><strong>Less com<\/strong><strong>mon causes are:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>intrabronchial lesion<\/li>\n\n\n\n<li>severe bronchiectasis<\/li>\n\n\n\n<li>malignancy<\/li>\n\n\n\n<li>interstitial lung disease.<\/li>\n<\/ul>\n\n\n\n<p><strong>Non-specific and refractory cough<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Non-specific cough is a chronic cough not associated with any of the cough pointers or cough-related diagnosis.&nbsp; Investigations with chest x-rays and spirometry are normal<\/li>\n\n\n\n<li>Refractory cough is a cough that persists after therapy.
