The service is based on the principles of the&nbsp;<strong>right care, from the right person, at the right time in the right place<\/strong>. The service offers a single point of access for patients with routine&nbsp;<strong><em>spinal pain, lumbar spine related leg symptoms\/ cervical&nbsp; spine related arm symptoms<\/em><\/strong> who require referral for their problem following failure to improve with initial primary care management. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Once within the service patients will be able to access all the multidisciplinary services that are required for their problem including physiotherapy, investigation, surgical opinion and pain clinic opinion. There will be no need for patients to return to their GP to seek additional referrals or investigations for the lumbar spine problem. Cervical spine pathway | Right Decisions<\/a><\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Who to refer:<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>All routine cases of cervical spine pain and cervical spine related arm symptoms who have not responded to initial primary care management within a 4-6 week period and&nbsp;do not have the presence of significant red flags nor have suspected inflammatory spinal pain \u2013 see sections serious cervical spine conditions and other considerations and inflammatory spinal pain<\/li>\n\n\n\n<li>Cervical Spine related arm symptoms-&nbsp; Consider earlier referral in cases intractable symptoms and\/ or developing motor deficit but without significant red flags\n<ul class=\"wp-block-list\">\n<li>Non-specific cervical spine pain in the absence of a dominant psychological component who have not received recent physiotherapy for their condition or who have received physiotherapy previously for their condition but;<ul><li>have experienced a substantial change in their presentation<\/li><\/ul><ul><li>require assistance with achieving a functional goal<\/li><\/ul><ul><li>wish further assessment and considerations of management options<\/li><\/ul><\/li>\n\n\n\n<li>have clinical features suggestive of an underlying specific cause for their cervical spine&nbsp; disability, no planned referral to another speciality for diagnosis or treatment of pain \u2013 see Chronic pain&nbsp;<a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/chronicpain\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Chronic Pain \u2013 RefHelp<\/strong><\/a>.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">How to refer<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>For all cases of routine cervical spine pain \/ spine related arm symptoms\u2013 refer via&nbsp;<strong>SCI Gateway Referral \u201cAHP-Physiotherapy\u201d then choose local site.<\/strong><\/li>\n\n\n\n<li>This single referral will gain access to the full multidisciplinary NHS Lothian integrated back pain pathway. Any subsequent investigation\/ surgical opinion\/ pain service referral will be made within the service<\/li>\n\n\n\n<li>Please include all relevant details on your referral to allow the appropriate initial triage of the patient. Initial triage will occur within MSK Physiotherapy and if appropriate will be passed to spinal advanced practice physiotherapy service.<\/li>\n\n\n\n<li>Please signpost patients to&nbsp;<a href=\"https:\/\/services.nhslothian.scot\/ibps\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Integrated Spinal Service Webpage<\/strong><\/a>&nbsp;for patient information.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Other information<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>For all routine cases of cervical spine pain and cervical spine related arm symptoms who meet the criteria under \u201cwho to refer\u201d, referral to other specialities including Orthopaedics, Neurosurgery, Neurology &amp; Rheumatology is not indicated.&nbsp;&nbsp;<\/strong>Once within the service patients will be able to access all the multidisciplinary services that are required for their problem including physiotherapy, investigation, surgical opinion and pain clinic opinion.
