Bridge Builders are volunteers who accompany patients who may benefit from extra support to attend and engage in their outpatient appointments.<\/p>\n\n\n\n<p>A Bridge Builder can provide support in many ways. They can:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Meet the person on-site before their appointment.<\/li>\n\n\n\n<li>Help them find their way around the hospital or clinic.<\/li>\n\n\n\n<li>Sit in the appointment and act as an \u201cextra pair of ears\u201d. This is completely optional and the volunteer can step out at any time.<\/li>\n\n\n\n<li>Spend some time after the appointment to talk things through and ensure understanding of any next steps.<\/li>\n\n\n\n<li>Report any concerns or questions back to the clinic directly.<\/li>\n<\/ul>\n\n\n\n<p><strong>CM &amp; SD 11\/02\/26<\/strong><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<h4 class=\"wp-block-heading\">Who can refer:<\/h4>\n\n\n\n<p>We accept referrals from all health care professionals in both Primary and Secondary Care. Patients can also self-refer. There is a <a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/Bridge-Builders-Programme-v1.1-PIL.pdf\" data-type=\"link\" data-id=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/Bridge-Builders-Programme-v1.1-PIL.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Patient Information Leaflet<\/a> that patients can be directed to or can be printed out.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Who to refer:<\/h4>\n\n\n\n<p>This programme is open to anyone who:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Would benefit from having a volunteer attend their outpatient appointment with them.<\/li>\n\n\n\n<li>Has an outpatient appointment within the Edinburgh area.<\/li>\n<\/ul>\n\n\n\n<p>Support may be particularly helpful for those who:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Feel anxious or unsure about attending an appointment.<\/li>\n\n\n\n<li>Are attending alone and would prefer some company.<\/li>\n\n\n\n<li>Are unsure about where to go or how to get to the right place.<\/li>\n\n\n\n<li>Would benefit from talking things through before and\/or after their appointment.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Who not to refer:<\/h4>\n\n\n\n<p>This programme is unable to support patients who require:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Personal care (e.g. assistance with toileting).<\/li>\n\n\n\n<li>Transport to and from their appointment.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">How to refer:<\/h4>\n\n\n\n<p>Please complete a <a href=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/Bridge-Builder-Referral-Form-December-2025.docx\" data-type=\"link\" data-id=\"https:\/\/apps.nhslothian.scot\/files\/sites\/2\/Bridge-Builder-Referral-Form-December-2025.docx\" target=\"_blank\" rel=\"noreferrer noopener\">referral form<\/a> and email it to: <a href=\"mailto:loth.bridgebuilders@nhs.scot\" target=\"_blank\" rel=\"noreferrer noopener\">loth.bridgebuilders@nhs.scot<\/a><\/p>\n\n\n\n<p>As a minimum, we require by email the patient\u2019s:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Full name<\/li>\n\n\n\n<li>Contact number<\/li>\n\n\n\n<li>Consent for Bridge Builders team to make contact.<\/li>\n<\/ul>\n\n\n\n<p>Patients can be referred to the programme while awaiting confirmation of their outpatient appointment date\/time. This includes explaining the programme, outlining the volunteers support and identifying any needs or adjustments. Patients are matched with volunteers based on availability and individual needs.
