Our focus is to work with patients and carers on symptom recognition, management of exacerbation or decompensation, and prevention of avoidable hospital admissions.<\/p>\n\n\n\n<p>The main conditions include-:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>COPD<\/strong><\/li>\n\n\n\n<li><strong>Bronchiectasis<\/strong><\/li>\n\n\n\n<li><strong>Pulmonary Fibrosis<\/strong><\/li>\n\n\n\n<li><strong>End stage Heart Failure<\/strong><\/li>\n\n\n\n<li><strong>Increasing frailty with existing Long-Term conditions<\/strong><\/li>\n<\/ul>\n\n\n\n<p>IMPACT supports patients and carers to understand their long-term conditions in order to optimise good health and wellbeing, make achievable lifestyle changes and improve medication concordance.<\/p>\n\n\n\n<p>Any health professional may refer to IMPACT. All referrals will be reviewed prior to acceptance<\/p>\n\n\n\n<p>Please ensure the patient has consented to the referral.<\/p>\n\n\n\n<p><strong>BC &amp; JD 22\/09\/2025<\/strong><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p><strong>Information to include with referral:<\/strong><\/p>\n\n\n\n<p>From acute services: hospital discharge letter with medication or IMPACT referral form<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Who to refer:<\/strong><\/h4>\n\n\n\n<p>Patients must<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Be registered with an Edinburgh GP and reside in Edinburgh<\/li>\n\n\n\n<li>Be aged over 16 years<\/li>\n\n\n\n<li>Have one or more long term condition that includes a respiratory or cardiac condition<\/li>\n\n\n\n<li>Be willing to engage with the service, or have someone able to engage on their behalf<\/li>\n<\/ul>\n\n\n\n<p><strong>Patients must also meet at least 2 of the following criteria<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>2 or more <strong>hospital admissions<\/strong>&nbsp;<strong>related to a respiratory or cardiac long-term condition&nbsp;<\/strong>in the past 12 months<\/li>\n\n\n\n<li>2 or more <strong>A&amp;E attendances<\/strong>&nbsp;<strong>related to a respiratory or cardiac long-term condition&nbsp;<\/strong>in the past 12 months<\/li>\n\n\n\n<li>Multiple recent exacerbations of a cardiac or respiratory condition not requiring either of the above<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Who not to refer:<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Unwilling or unable to engage with IMPACT&nbsp;<\/li>\n\n\n\n<li>Where patients are approaching&nbsp;end of life care they should already be known to their district nursing team.
