{"id":24388,"date":"2025-06-19T13:04:49","date_gmt":"2025-06-19T12:04:49","guid":{"rendered":"https:\/\/apps.nhslothian.scot\/refhelp\/?page_id=24388"},"modified":"2025-06-23T09:57:39","modified_gmt":"2025-06-23T08:57:39","slug":"eating-disorders-adult","status":"publish","type":"page","link":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/mentalhealthadult\/eating-disorders-adult\/","title":{"rendered":"Eating Disorders (Adult)"},"content":{"rendered":"\n<p>The Lothian Eating Disorders Service (LEDS) is an outpatient service based at the Cullen Centre, close to the Royal Edinburgh Hospital.&nbsp; The service also makes onward referrals for inpatient treatment at the Regional Eating Disorders Service (REDU) based at St John\u2019s Hospital.&nbsp;<\/p>\n\n\n\n<p>Appointments usually take place at the Cullen Centre or virtually, via Near Me.<\/p>\n\n\n\n<p>Urgent referrals will be offered an assessment appointment within 2 \u2013 8 weeks by the Cullen Centre if they meet criteria for a primary eating disorder diagnosis.<\/p>\n\n\n\n<p>LEDS offers:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Medical and psychiatric assessment, monitoring and review<\/li>\n\n\n\n<li>Dietetic assessment and follow up<\/li>\n\n\n\n<li>Individual and group therapy (including Cognitive Behavioural Therapy, Cognitive Analytic Therapy, Interpersonal Therapy, and Schema Therapy)<\/li>\n\n\n\n<li>Guided self-help and psychoeducation<\/li>\n\n\n\n<li>Direct support (e.g. with eating, welfare advice, developing social networks) &nbsp;<\/li>\n\n\n\n<li>Facilitated self-help group work<\/li>\n\n\n\n<li>Carer and family support&nbsp;<\/li>\n\n\n\n<li>Home and community visits if it is not medically safe for the patient to travel<\/li>\n\n\n\n<li>Liaison advice and consultation for primary and secondary care<\/li>\n\n\n\n<li>A medical and dietetic liaison service, specific to eating disorders, to acute medical services<\/li>\n\n\n\n<li>Referral to inpatient services at the Regional Eating Disorders Unit (REDU). REDU is located in St John\u2019s Hospital, Livingston. Further information about services can be found on the <a href=\"https:\/\/services.nhslothian.scot\/seeds\/regional-eating-disorder-unit\/\" target=\"_blank\" rel=\"noreferrer noopener\">REDU webpage<\/a>.<\/li>\n<\/ul>\n\n\n\n<p>The Outpatient Team specialises in Cognitive Behavioural Therapy but can also offer a variety of other psychotherapeutic techniques dependent on individual needs. The centre adopts a matched care approach, matching the intensity and model of therapy to individual clinical needs.<\/p>\n\n\n\n<p>People under the age of 18 should be referred to&nbsp;the <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/childandadolescentmentalhealthcamh\/eatingdisorderscahms\/\" target=\"_blank\" rel=\"noreferrer noopener\">CAMHS ED service<\/a>. &nbsp;<\/p>\n\n\n\n<p><em>If there is uncertainty about suitability of a referral, please telephone the referrals co-ordinator on 0131 537 6655 for a preliminary discussion.<\/em><\/p>\n\n\n\n<p>For medical liaison (referrers to use if they want advice about an investigation or if the patient is ill but refuses referral), please e-mail: <a href=\"mailto:loth.cullenadmin@nhs.scot\" target=\"_blank\" rel=\"noreferrer noopener\">loth.cullenadmin@nhs.scot<\/a> and your query will be directed to the relevant professional.<\/p>\n\n\n\n<p><strong><u>DIAGNOSTIC CRITERIA<\/u><\/strong><\/p>\n\n\n\n<p>Please see the diagnostic criteria for eating disorders, <em>but note that patients with eating disorders may not have all the symptoms<\/em>:<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><tbody><tr><td><strong>\u200bAnorexia Nervosa<\/strong><\/td><td>\u200b<strong>Bulimia Nervosa<\/strong><\/td><td><strong>Binge Eating Disorder<\/strong><\/td><td><strong>Avoidant Restrictive Food Intake Disorder (ARFID)<\/strong> <strong>&nbsp;<\/strong><\/td><\/tr><tr><td>\u200bSignificantly low body weight (e.g. BMI&lt;18.5kg\/m2) &nbsp; <br><br>Marked distress about the pattern of binge eating and compensatory behaviours<\/td><td>Marked distress or significant functional impairment due to the pattern of binge eating<\/td><td>&nbsp;<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<h4 class=\"wp-block-heading\">W<strong>ho can refer:<\/strong><\/h4>\n\n\n\n<p>Referrals are accepted from GPs, Community Mental Health Teams, or other medical or mental health professionals.<\/p>\n\n\n\n<p><em>Where a patient is already known to a mental health service, referral should come from that service.&nbsp;<\/em><\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Who to refer:<\/strong><\/h4>\n\n\n\n<p>Referrals are accepted for all those aged 18 or over with eating disorder diagnoses including:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Anorexia nervosa (AN)<\/li>\n\n\n\n<li>Binge eating disorder (BED)<\/li>\n\n\n\n<li>Bulimia nervosa (BN)<\/li>\n\n\n\n<li>Other specified feeding and eating disorder (OFSED)<\/li>\n\n\n\n<li><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/avoidant-restrictive-food-intake-disorder-arfid\/\" target=\"_blank\" rel=\"noreferrer noopener\">Avoidant Restrictive Food Intake Disorder (ARFID)<\/a><\/li>\n<\/ul>\n\n\n\n<p><strong><u>Specialist Eating Disorder Unit (SEDU) admission<\/u><\/strong><\/p>\n\n\n\n<p>All referrals to the Regional Eating Disorder Unit (REDU) at St John\u2019s Hospital are made through LEDS.&nbsp; If admission is considered likely, please refer to LEDS in the first instance.<\/p>\n\n\n\n<p class=\"has-alert-red-color has-text-color has-link-color wp-elements-12f051861cf527fb0c01ee17e9be90bd\"><strong><em>Patients requiring emergency medical treatment linked to an eating disorder should be referred to acute medical services<\/em><\/strong><em> <strong>as well as being referred urgently to LEDS.<\/strong>&nbsp;<\/em><\/p>\n\n\n\n<p class=\"has-alert-red-color has-text-color has-link-color wp-elements-70a4bfcd652e87be072fa33705c61dc3\"><strong><em>Patients requiring urgent referral because of their psychiatric risk need to be referred to general adult psychiatry\/crisis services as well as LEDS.<\/em><\/strong><em> <\/em><\/p>\n\n\n\n<p>They will be offered a routine assessment appointment by the Cullen Centre if they also meet criteria for eating disorder treatment.<\/p>\n\n\n\n<p class=\"has-alert-red-color has-text-color has-link-color wp-elements-dd923a3eff03f96a4e891d7d735c873d\"><strong>If the person is ill, but refuses referral, please e-mail: <\/strong><\/p>\n\n\n\n<p><a href=\"mailto:loth.cullenadmin@nhs.scot\" target=\"_blank\" rel=\"noreferrer noopener\">loth.cullenadmin@nhs.scot<\/a> <strong>and your query will be directed to the relevant professional.<\/strong><\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Who not to refer:<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Simple obesity (see the <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/dietetics\/weightmanagementserviceadult\/\" target=\"_blank\" rel=\"noreferrer noopener\">Weight Management Service<\/a>)<\/li>\n\n\n\n<li>Patients with a primary personality disorder or addiction or severe mental health problem who also have bulimia or binge eating disorder should initially be referred to general adult or addiction services for assessment.<\/li>\n\n\n\n<li>ARFID where physical co-morbidities or GI symptoms have not been assessed and managed \u2013 see <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/avoidant-restrictive-food-intake-disorder-arfid\/\" target=\"_blank\" rel=\"noreferrer noopener\">ARFID<\/a> for more detail.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>How to refer:<\/strong><\/h4>\n\n\n\n<p>Referrals should be via SCI Gateway.&nbsp; beating eating disorders website<\/strong>&nbsp;<\/a> provides help on all aspects of eating disorders including Anorexia Nervosa, Bulimia Nervosa, Binge Eating Disorder and related eating disorders.
