{"id":21592,"date":"2024-11-19T11:07:34","date_gmt":"2024-11-19T11:07:34","guid":{"rendered":"https:\/\/apps.nhslothian.scot\/refhelp\/?page_id=21592"},"modified":"2026-04-17T08:45:16","modified_gmt":"2026-04-17T07:45:16","slug":"marie-curie-lothian-nursing-hospicehome-services","status":"publish","type":"page","link":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/palliativecare\/marie-curie-lothian-nursing-hospicehome-services\/","title":{"rendered":"Marie Curie Lothian Nursing Hospice@Home Services"},"content":{"rendered":"\n<p><strong>Information<\/strong><\/p>\n\n\n\n<p><strong><u>Hospice Care @ Home Daytime Service (Fast track)<\/u><\/strong><\/p>\n\n\n\n<p>Provides personal and emotional care to patients and their families in their own home for up to two visits during the day\/evening.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Patients can be referred from any health care professional but must be&nbsp;known to their District Nurse team.&nbsp;<\/li>\n\n\n\n<li>&nbsp;Patients can be referred to the service for&nbsp;end of life care, carer\/family distress, or urgent palliative care need\/crisis.&nbsp;<\/li>\n\n\n\n<li>Referrals are prioritised and care allocated by Registered Nurses as appropriate.<\/li>\n\n\n\n<li>This service is provided by Health Care Assistants.&nbsp;&nbsp;<\/li>\n\n\n\n<li>Shift times are from 8am \u2013 9pm.<\/li>\n\n\n\n<li>Caseloads will be reviewed by a Registered Nurse and patients may be discharged.<\/li>\n\n\n\n<li>Rereferrals will be considered.<\/li>\n<\/ul>\n\n\n\n<p><a><strong><u>Hospice&nbsp;Care @ Home Overnight Service (Managed Care)<\/u><\/strong><\/a><\/p>\n\n\n\n<p>Provides nursing, personal and emotional care to patients and their families in their own home<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Patients can be referred to the service for&nbsp;end of life care, carer\/family distress within the last six months of life.<\/li>\n\n\n\n<li>Referrals are prioritised for care using a prioritisation tool and care allocated by Registered Nurses as appropriate.<\/li>\n\n\n\n<li>This service is predominantly provided over night from 10pm until 7am by HCAs with some daytime Registered Nurse capacity as appropriate.<\/li>\n\n\n\n<li>Patients must be known to their District Nursing team<\/li>\n<\/ul>\n\n\n\n<p><strong><u>Urgent Hospice Care @ Home (Rapid Response)<\/u><\/strong><\/p>\n\n\n\n<p>Provides responsive person-centred care at home throughout the night 21:00 \u2013 09:00<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Two teams (one Registered Nurse and One Health Care Assistants) work closely with LUCS to prevent crisis admissions, provide symptom management and support patients and their families.<\/li>\n\n\n\n<li>Urgent visit requests come&nbsp;via the LUCS team (DNs, GPs), Scottish Ambulance Paramedics, Care Home staff or directly&nbsp;from patients who have previously accessed the service or are known to one of the Lothian community specialist teams.<\/li>\n\n\n\n<li>Contact number has been provided to all care homes in Lothian offering support and visit overnight if appropriate.<\/li>\n<\/ul>\n\n\n\n<p><strong>M.A &amp; T.D. 19-11-24<\/strong><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<h4 class=\"wp-block-heading\"><strong>Who can refer:<\/strong><\/h4>\n\n\n\n<p>Referrals can be submitted via patient\u2019s district nursing team but can also be done by another healthcare professional (ideally in discussion with the DN team)<\/p>\n\n\n\n<p>Rapid Response urgent visits can be requested by LUCS team (DNs, GPs), Scottish Ambulance Paramedics, Care Home staff, or directly&nbsp;from patients who have previously accessed the service or are known to one of the Lothian community specialist teams.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Who to refer:<\/strong><\/h4>\n\n\n\n<p><strong><u>Daytime Service (Fast track): <\/u><\/strong>Patients can be referred to the service for&nbsp;end of life care, carer\/family distress, or urgent palliative care need \/ crisis.&nbsp; This is a short term service up to 14days<\/p>\n\n\n\n<p><strong><u>Overnight Service (Managed Care)<\/u><\/strong> Patients can be referred to the service for&nbsp;end of life care, carer\/family distress within the last six months of life.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Who not to refer:<\/strong><\/h4>\n\n\n\n<p>For referrals to Overnight Service (Managed Care),patients must be known to their DN team<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>How to refer:<\/strong><\/h4>\n\n\n\n<p>Referrals can be discussed with the Scotland co-ordination Hub by calling 0141 473 1785.
