{"id":21531,"date":"2024-11-14T12:01:10","date_gmt":"2024-11-14T12:01:10","guid":{"rendered":"https:\/\/apps.nhslothian.scot\/refhelp\/?page_id=21531"},"modified":"2024-12-27T11:56:52","modified_gmt":"2024-12-27T11:56:52","slug":"marie-curie-edinburgh-hospice-services","status":"publish","type":"page","link":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/palliativecare\/marie-curie-edinburgh-hospice-services\/","title":{"rendered":"Marie Curie Edinburgh hospice services"},"content":{"rendered":"\n<p><strong>Information<\/strong><\/p>\n\n\n\n<p><strong>Marie Curie Hospice\u2019s services are for those living in South\/Southeast Edinburgh, Midlothian and West Lothian only.<\/strong><strong><\/strong><\/p>\n\n\n\n<p><strong><u>Single Point of Access <\/u><\/strong><\/p>\n\n\n\n<p><strong>Provides a single point of contact and access for all hospice services\/support <\/strong><strong><\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Responds to requests for advice and information from patients, families, health care professionals and other services<\/li>\n\n\n\n<li>Triage of all new referrals<\/li>\n\n\n\n<li>Specialist telephone assessment to determine ongoing needs and appropriate service for referral.<\/li>\n\n\n\n<li>Manages waiting lists and admissions to all the specialist hospice services\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/palliativecare\/marie-curie-edinburgh-hospice-services\/inpatient-care\/\" target=\"_blank\" rel=\"noreferrer noopener\">Inpatient care<\/a><\/li>\n\n\n\n<li><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/palliativecare\/marie-curie-edinburgh-hospice-services\/community-specialist-palliative-care-team\/\" target=\"_blank\" rel=\"noreferrer noopener\">Community specialist palliative care team &nbsp;<\/a><\/li>\n\n\n\n<li><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/palliativecare\/marie-curie-edinburgh-hospice-services\/enhanced-hospice-care-home\/\" target=\"_blank\" rel=\"noreferrer noopener\">Enhanced Hospice Care @ Home<\/a><\/li>\n\n\n\n<li><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/palliativecare\/marie-curie-edinburgh-hospice-services\/wellbeing-outpatients\/\" target=\"_blank\" rel=\"noreferrer noopener\">Wellbeing &amp; Outpatients<\/a><\/li>\n\n\n\n<li><a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/palliativecare\/marie-curie-edinburgh-hospice-services\/patient-family-support-team\/\" target=\"_blank\" rel=\"noreferrer noopener\">Patient and Family Support Team<\/a><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<p><strong>M.A &amp; T.D. 14-11-24<\/strong><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p><strong><u>All referrals are managed by the Single Point of Access team<\/u><\/strong><\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Who can refer:<\/strong><\/h4>\n\n\n\n<p>Any healthcare professional can refer although we request that the patient\u2019s own GP is aware of the referral<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Who to refer:<\/strong><\/h4>\n\n\n\n<p>Patients over 16 years (and their families and informal carers) with complex problems associated with active advanced and progressive illness, irrespective of underlying diagnosis.<\/p>\n\n\n\n<p>\u2018Complex problems\u2019 are defined as those which are severe and unable to be managed by current care team or setting.
