{"id":18168,"date":"2024-02-06T15:38:22","date_gmt":"2024-02-06T15:38:22","guid":{"rendered":"https:\/\/apps.nhslothian.scot\/refhelp\/?page_id=18168"},"modified":"2024-12-27T12:07:43","modified_gmt":"2024-12-27T12:07:43","slug":"st-columbas-care-at-home","status":"publish","type":"page","link":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/palliativecare\/st-columbas-hospice\/st-columbas-care-at-home\/","title":{"rendered":"St Columba\u2019s Care at Home\u00a0"},"content":{"rendered":"\n<p>Care at home is delivered by experienced hospice at home teams across North Edinburgh and East Lothian.<\/p>\n\n\n\n<p>The Care at Home service offers support to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>prevent of admission to or facilitate discharge from inpatient care<\/li>\n\n\n\n<li>manage crisis<\/li>\n\n\n\n<li>support at times of rapid change including end of life care<\/li>\n\n\n\n<li>offer respite support for families and carers<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>promote independence through enabling, empowering, and educating patients and families to be where they want to be and live as they choose for as long as they can.<\/li>\n<\/ul>\n\n\n\n<p>The service is delivered over 7 days from 8am-8pm, with up to 3 daily visits for period of up to 8 weeks.
