{"id":17726,"date":"2023-12-12T12:27:08","date_gmt":"2023-12-12T12:27:08","guid":{"rendered":"https:\/\/apps.nhslothian.scot\/refhelp\/?page_id=17726"},"modified":"2023-12-19T13:20:32","modified_gmt":"2023-12-19T13:20:32","slug":"recurrent-oral-herpes-simplex-virus-hsv-infection","status":"publish","type":"page","link":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/infectiousdiseases\/recurrent-oral-herpes-simplex-virus-hsv-infection\/","title":{"rendered":"Recurrent Oral Herpes Simplex Virus (HSV) Infection"},"content":{"rendered":"\n<p>Prophylactic oral antiviral treatment should be considered if the patient is:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>immunocompromised and has troublesome recurrent oral herpes simplex infection<\/li>\n\n\n\n<li>has&nbsp;frequent (\u2265 6 or more episodes in one year), persistent and\/or&nbsp;severe episodes of recurrent&nbsp;oral herpes simplex infection<\/li>\n\n\n\n<li>has herpes simplex associated with recurrent erythema multiforme.<\/li>\n<\/ul>\n\n\n\n<p>Those with atypical lesions or in whom the diagnosis is uncertain (but thought to be benign) &nbsp;should be referred routinely to <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/dentistryoralhealthservice\/oralmedicine\/\" target=\"_blank\" rel=\"noreferrer noopener\">Oral Medicine<\/a>. Patients with unexplained ulceration of the oral mucosa persisting &gt; 3 weeks should be referred to <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/oralmaxillofacialsurgery\/\" target=\"_blank\" rel=\"noreferrer noopener\">Oral and Maxillofacial Surgery<\/a> \u2013 urgent, suspicious of cancer.<\/p>\n\n\n\n<p>Severe or recurrent genital HSV infection should be referred to <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/sexuallytransmittedinfectionssti\/\" target=\"_blank\" rel=\"noreferrer noopener\">Sexual Health services<\/a>.<\/p>\n\n\n\n<p><strong>C.M &amp; N.B 12-12-23<\/strong><\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<h4 class=\"wp-block-heading\"><strong>Who to refer:<\/strong><\/h4>\n\n\n\n<p>Patients with confirmed non-genital HSV infection which is refractory to oral antiviral therapy. &nbsp;<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Who not to refer:<\/strong><\/h4>\n\n\n\n<p>Patients with<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>atypical lesions or where the diagnosis is uncertain, but thought to be benign \u2013 refer to Oral Medicine<\/li>\n\n\n\n<li>unexplained ulceration of the oral mucosa persisting &gt; 3 weeks \u2013 refer to OMFS as urgent, suspicious of cancer.<\/li>\n\n\n\n<li>severe or recurrent genital HSV infection \u2013 refer to <a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/sexuallytransmittedinfectionssti\/\" target=\"_blank\" rel=\"noreferrer noopener\">Sexual Health services<\/a>.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>How to refer:<\/strong> <\/h4>\n\n\n\n<p>Via Sci Gateway to Infectious Diseases at the Western General Hospital<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Primary Care Management<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<p>Guidance on the management of uncomplicated oral HSV is available as a NICE <a href=\"https:\/\/cks.nice.org.uk\/topics\/herpes-simplex-oral\/\" target=\"_blank\" rel=\"noreferrer noopener\">CKS summary<\/a>. &nbsp;Patients should be offered screening for HIV infection.
