This is an outpatient service, which may involve one or several attendances, scheduled weekly.&nbsp; It is run between 9 &#8211; 4pm Monday to Friday.&nbsp; Patients attend for the whole morning, to early or mid afternoon (sandwich lunch provided) and return visits are scheduled as necessary. &nbsp;<\/p>\n\n\n\n<p>Limited ambulance transport is available to and from appointments for those who need it.<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table><tbody><tr><td>Assessment\/treatment for complex medical problems<\/td><td><strong>Available Resources:<\/strong>                                                                                                                                              <\/td><\/tr><tr><td>Specialist medical and nursing procedures e.g. blood transfusion, zolendronate infusion, leg ulcer treatment<\/td><td>Medical team<\/td><\/tr><tr><td>Crisis intervention to prevent hospital admission<\/td><td>Nursing team<\/td><\/tr><tr><td>Management of complex long term conditions e.g. heart failure, COPD, leg ulcers, Parkinson\u2019s disease<\/td><td>Physiotherapy<\/td><\/tr><tr><td>Falls assessment and prevention programme<\/td><td>Occupational therapy<\/td><\/tr><tr><td>Rehabilitation<\/td><td>Speech and language therapy<\/td><\/tr><tr><td>Supported discharge of hospital inpatients<\/td><td>Diagnostics e.g. Blood tests, Radiology: plain x-ray, ultrasound, CT , ECG, echocardiogram, 24 hour ambulatory ECG, pulmonary function testing, ABPI Doppler&#8217;s, bladder scanning, 24 hour BP monitoring<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p><strong>Consultants <\/strong>Dr Jo Cowell, Dr Conor Maguire, Dr John Mair<\/p>\n\n\n\n<p><strong>Speciality Doctor <\/strong>Dr Inas Saad<\/p>\n\n\n\n<div class=\"wp-block-getwid-tabs\" data-active-tab=\"0\"><ul class=\"wp-block-getwid-tabs__nav-links\"><\/ul>\n<div class=\"wp-block-getwid-tabs__nav-link\"><span class=\"wp-block-getwid-tabs__title-wrapper\"><a href=\"#\"><span class=\"wp-block-getwid-tabs__title\">Referral Guidelines<\/span><\/a><\/span><\/div><div class=\"wp-block-getwid-tabs__tab-content-wrapper\"><div class=\"wp-block-getwid-tabs__tab-content\">\n<h4 class=\"wp-block-heading\">Who to refer:<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Patients with acute reduction in mobility or function<\/li>\n\n\n\n<li>Frequent (\u22652 in 6 months) or unexplained falls<\/li>\n\n\n\n<li>Those for whom urgent Day Hospital attendance might avoid hospital admission<\/li>\n\n\n\n<li>Patients requiring medical AND nursing or AHP input<\/li>\n\n\n\n<li>Those with Parkinson&#8217;s, stroke, osteoarthritis, osteoporosis and or mobility or multiple medical problems<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Who not to refer:<\/h4>\n\n\n\n<p>Unstable clinical condition e.g.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Acute stroke\/TIA<\/li>\n<\/ul>\n\n\n\n<p>Suspected DVT or PE<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>GI haemorrhage<\/li>\n<\/ul>\n\n\n\n<p>Chest pain\/acute cardiac problems<\/p>\n\n\n\n<p>Patients who are unable to participate in treatment or rehabilitation<\/p>\n\n\n\n<p>Patients whose needs can be addressed by community services e.g. only requiring physiotherapy<\/p>\n\n\n\n<p>Patients aged under 65 years (unless discussed with consultant first)<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>How to refer:<\/strong><\/h4>\n\n\n\n<p><strong>Routine referrals<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>SCI Gateway<\/li>\n<\/ul>\n\n\n\n<p><strong>Urgent referrals<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>SCI Gateway<\/li>\n<\/ul>\n\n\n\n<p>For advice or discussion about urgent referrals, please contact the Consultant on call for MoE at WGH:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Geriatric Medicine Advice Hotline: 0131 465 9107 (08.30 &#8211;
