{"id":15081,"date":"2023-05-09T11:10:52","date_gmt":"2023-05-09T10:10:52","guid":{"rendered":"https:\/\/apps.nhslothian.scot\/refhelp\/?page_id=15081"},"modified":"2026-03-30T12:16:41","modified_gmt":"2026-03-30T11:16:41","slug":"out-of-hours-hh","status":"publish","type":"page","link":"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/admissionsurgentcare\/edinburgh-hscp\/out-of-hours-hh\/","title":{"rendered":"Out Of Hours H@H"},"content":{"rendered":"\n<p><strong>OOH LUCS referrals to Lothian Hospital at Home Teams<\/strong><\/p>\n\n\n\n<p>Secondary level acute medical care for people who would otherwise require admission.<\/p>\n\n\n\n<p><br><strong>Edinburgh, Midlothian, East Lothian \u2013 Out of Hours<\/strong><\/p>\n\n\n\n<p>Referrals accepted: Mon-Sun 8pm \u2013 8am<\/p>\n\n\n\n<p><br><strong>West Lothian \u2013 Out of Hours<\/strong><\/p>\n\n\n\n<p>Referrals accepted: Mon &#8211; Fri 8pm \u2013 8am<\/p>\n\n\n\n<p>Weekends 6pm \u2013 8am<\/p>\n\n\n\n<p><br><strong>Overnight: <\/strong>Total 3 patients can be placed on workbench for review next morning, for <strong>each<\/strong> locality.<\/p>\n\n\n\n<p><br><strong>Contact and referral details \u2013 ALL LOCALITIES<\/strong><\/p>\n\n\n\n<p>Flow Navigation Centre: 03000 134 000 (option 1 then option 4)<\/p>\n\n\n\n<p><br><strong>Referral criteria<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Resident within any NHS Lothian locality&nbsp; &nbsp;<\/li>\n\n\n\n<li>Patient, family and referrer should be willing for treatment at home<\/li>\n\n\n\n<li>&gt; 65\u2019s with frailty (including delirium and dementia), comorbidity (decompensation of long-term conditions) and those with complex or palliative needs who would otherwise need admission to hospital<\/li>\n\n\n\n<li>&lt; 65 with acute decompensation of long-term conditions and\/or complex needs including physical and\/or learning disability and neurodegenerative conditions<\/li>\n\n\n\n<li>Hospital at Home will assess safety at home and liaise with therapy and social care input as indicated.<\/li>\n<\/ul>\n\n\n\n<p><br><strong>Exclusion criteria:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Cardiac chest pain<\/li>\n\n\n\n<li>Acute stroke<\/li>\n\n\n\n<li>Acute Asthma<\/li>\n\n\n\n<li>Suspected DVT\/PE<\/li>\n\n\n\n<li>Trauma including suspected fracture(s)<\/li>\n\n\n\n<li>Acute surgical emergencies<\/li>\n\n\n\n<li>New oxygen requirement &gt;4L<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Please ensure Adastra referral is selected with appropriate H@H team, and \u201creferred to H@H\u201d selected in outcome.<\/h4>\n\n\n\n<p><strong>J.B &amp; J.J &#8211; 30-3-26<\/strong><\/p>\n","protected":false},"excerpt":{"rendered":"<p>OOH LUCS referrals to Lothian Hospital at Home Teams Secondary level acute medical care for people who would otherwise require admission.
